Full Job Description

Access Ohio Behavioral Health is seeking Full-Time Case Managers to support our patients for several of our Columbus area offices. Case Managers are a vital part of our interdisciplinary team and work to ensure continuity of care, community connection and lasting recovery. This is a field-based position with a 70% productivity standard and requires at least 80% of services to be provided to consumers in their homes, shelters, places of employment, vocational and education settings and other community settings.

Schedule & Pay Scale:

- Full-Time M-F, 40 Hours per Week (no weekends).

- HS Diploma/Associates- $19-$21 per hour

-Bachelors/Masters- $20-$23 per hour

Qualifications:

-Master’s and Bachelor’s degree in social work/psychology OR associates degree in human services/substance abuse preferred. Will consider experienced case managers with demonstrated work performance and no degree.

-Previous experience in Community Based Behavioral Health, Addiction and Recovery required.

Key Responsibilities:

-Provide case management services for a caseload of clients with mental health and/or substance abuse diagnosis.

-Conduct initial needs assessments and develop individualized care plans.

-Assist clients with accessing community resources, including housing, food security and healthcare.

-Collaborate with therapists, physicians and other team members.

-Maintain accurate documentation of services, progress notes and communications in accordance with agency and licensing standards.

-Attend staff meetings and trainings as required.

-Provide education and advocacy to support client stability and self-sufficiency whether in office or out in the community.

Requirements:

Valid Ohio Driver’s License with no more than 2 points
Reliable transportation and willingness to transport clients.
Current automobile liability insurance.
Clean urine drug screen upon hire (zero tolerance policy).
Background check upon hire.
Obtain and approved Ohio Department of Medicaid provider agreement and updated to include agency-level association within 3 days of hire.

Pay: $19.00 - $23.00 per hour
