Enliven Home Health – East Columbus, Ohio

Position Type: Part-Time / Per Visit (PRN Available) 10-20 points per week 10-30 hrs.

Service Area: East Side. Gahanna, Pickerington, Canal Winchester, Groveport

About Enliven Home Health

Enliven Home Health is dedicated to helping patients maintain independence and improve quality of life by providing exceptional therapy services where they live. Approximately 80–90% of our patients reside within assisted living and independent senior living communities, allowing therapists to enjoy efficient scheduling, minimal drive time, and the opportunity to build meaningful relationships with residents and community staff.

Position Summary

Enliven Home Health is seeking a compassionate, motivated Occupational Therapist to provide skilled home health services to adults and older adults. The Occupational Therapist evaluates patients, develops individualized treatment plans, and delivers evidence-based interventions to maximize independence, safety, and participation in meaningful daily activities.

Because the majority of our patients are located within senior living communities, therapists benefit from treating multiple patients in one location while collaborating closely with nursing staff, caregivers, and interdisciplinary team members.

Responsibilities

Perform comprehensive occupational therapy evaluations.
Develop and implement individualized treatment plans.
Provide interventions to improve:
Activities of Daily Living (ADLs)
Instrumental Activities of Daily Living (IADLs)
Upper extremity strength and coordination
Balance and fall prevention
Home and environmental safety
Energy conservation and adaptive techniques
Recommend adaptive equipment and train patients in proper use.
Educate patients, families, and caregivers.
Collaborate with physicians, nurses, physical therapists, speech therapists, and community staff.
Complete timely and accurate documentation within the electronic medical record.
Participate in care conferences and interdisciplinary communication.

Qualifications

Graduate of an accredited Occupational Therapy program.
Current Ohio Occupational Therapy license.
Current CPR certification (or willingness to obtain).
Valid driver’s license and reliable transportation.
Home health experience preferred but not required.
Experience working with geriatric populations is highly desirable.
Strong communication and time-management skills.
