Overview

Join our dynamic healthcare team as a Home Health Physical Therapist, where you will play a vital role in delivering personalized rehabilitative care directly to patients in their homes. This energetic position offers the opportunity to make a meaningful impact on patients’ recovery journeys, especially those in geriatric, pediatric, neurological, orthopedic, and inpatient settings. You will collaborate with a multidisciplinary team to develop and implement effective treatment plans that promote independence and improve quality of life. Duties

Conduct comprehensive patient assessments to determine physical therapy needs and establish individualized care plans.
Deliver targeted therapy interventions such as therapeutic exercises, manual therapy, trigger point therapy, and mobility training tailored to each patient’s condition.
Develop and update treatment plans based on ongoing patient progress and clinical observations, ensuring adherence to medical terminology and physiotherapy standards.
Collaborate closely with healthcare professionals including physicians, nurses, occupational therapists, and social workers to coordinate patient care and discharge planning.
Manage clinical documentation with precision, maintaining accurate records in electronic health records (EHR) systems and ensuring compliance with healthcare regulations.
Educate patients and their families on home exercise programs, safety measures, and community resources to support ongoing recovery at home.
Perform discharge planning by assessing patient progress and facilitating smooth transitions from active therapy to community or outpatient care.

Experience

Proven experience practicing in a physical therapy role within home health or outpatient clinics, with a strong focus on patient rehabilitation across diverse populations including geriatrics and pediatrics.
Demonstrated expertise in acute care environments such as hospitals or inpatient units, with familiarity in inpatient experience preferred.
Hands-on experience developing treatment plans for orthopedic, neurological, and musculoskeletal conditions using physiotherapy techniques.
Knowledge of anatomy, physiology, medical terminology, and clinical documentation standards necessary for effective patient care delivery.
Skilled in working with electronic health records (EHR) or electronic medical records (EMR) systems for documentation management.
Ability to communicate clearly with clients and healthcare teams while demonstrating compassion for patients’ needs in home & community care settings.
Experience working within outpatient rehabilitation centers or outpatient clinics is highly valued; familiarity with outpatient experience enhances your ability to adapt therapies effectively.
