Full Job Description

POSITION DEFINITION

This position serves on a multi-disciplinary team that provides wraparound services for youth and family in the community setting. This position will be responsible for providing crisis intervention and long-term case management services for at risk youth and their families in the community, while collaborating with community partners (Recreation and Parks, Columbus City Schools, etc). It will provide assessment, screening, care planning, brief therapy sessions, linkage to resources, as well as completing documentation. Additionally, this position will provide education on trauma and resiliency. Evening and weekend hours are required.

PREFERRED QUALIFICATIONS

Master’s Degree in Social Work, with a Social Work License (LSW) or Master’s in Counseling, with a Clinical Counselors License (LPC) preferred. Experience in crisis intervention and working with youth, families and vulnerable communities; strong understanding of trauma; strong communication, clinical skills, and public speaking skills; and the ability to work with people from different backgrounds and experience with group facilitation, community outreach and community engagement preferred. Experience with Microsoft Word, Outlook, PowerPoint and Excel highly preferred.

ESSENTIAL FUNCTIONS

Resilience Support: Provide psychosocial intervention including assessment, care planning, adolescent interventions and family interventions. Partner with a Coordination Assistant and Community Interventionist (CRPD) for home, school, and any community visits. Co-facilitating client meetings, case prep work, following up with community partners, adolescents, parents, and conducting group sessions when necessary. Locates and maintains listing of relevant resources within the community to support trauma interruption. Assist with community-based support groups in schools, faith-based organizations, and community centers.

Reporting and Documentation: Complete documentation in the Electronic Health Record. Complete assigned case and monthly reports accurately and timely.

ReRoute Representative: Represents Columbus Public Health and ReRoute at meetings, community events, and all ReRoute initiatives as assigned. Attends and participates in ReRoute staff meetings, workshops and youth outings. Participate in meetings as specified by supervisor, including staff/departmental meetings. Support Neighborhood Social Services workgroups, meetings, and counseling needs.
