Full Job Description

Now Hiring: Case Management Coordinator / Social Work (Child / Youth Behavioral Health)
Pay Rate: $27.17 per hour
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (flexibility required to meet member needs)
Travel: Up to 50% within assigned region + 1–2 onsite meetings per year in Franklin County

This is a full-time, field-based telework position serving members across Northeast Ohio. Candidates must reside in one of the following counties:

Ashland, Richland, Wayne, Fayette, Highland, Ross, Pike, Jackson, Licking, Muskingum, Coshocton, Fairfield, Franklin, Morrow, Washington

Candidates may be required to travel 1–2 times per year to the corporate office in New Albany (Franklin County) for onsite meetings and trainings.

This role plays a critical part in improving outcomes for children and families through collaboration, coordination, and community-based engagement.

Position Summary

The Case Management Coordinator uses critical thinking and clinical judgment to:

Conduct comprehensive member evaluations
Develop and coordinate individualized care plans
Support children and families across multiple systems
Promote access to medically appropriate, high-quality behavioral health services
Serve as a single point of contact for members

This role requires strong community engagement and in-person visits to member homes and other locations throughout Northeast Ohio.

Fundamental Responsibilities

Evaluation of Members

Conduct comprehensive needs assessments using care management tools
Identify high-risk factors and service gaps
Recommend appropriate interventions and referrals
Implement and monitor individualized care plans

Care Coordination & Quality Improvement

Collaborate with Child & Family Teams
Consult with supervisors, Medical Directors, and interdisciplinary teams
Present cases in multidisciplinary conferences
Identify and escalate quality-of-care concerns
Utilize motivational interviewing and negotiation skills
Empower families to actively participate in healthcare decisions

Monitoring & Documentation

Maintain compliance with regulatory and accreditation standards
Accurately document case management activities
Track progress and adjust plans as needed

Required Qualifications

Bachelor’s degree or non-licensed master’s level clinician (behavioral health, human services, health services, public health preferred — e.g., psychology, social work, counseling, marriage & family therapy, juvenile justice)
2+ years of experience in:
Behavioral health, social services, or human services
Children’s mental health, child welfare, developmental disabilities, or juvenile justice
Community-based services for children and families
2+ years of experience with:
Case management, family systems, community resources, child protection, or child development
Ohio delivery systems and local community networks
MS Office Suite (Outlook, Word, Excel, SharePoint)
Reliable transportation (mileage reimbursed per company policy)
Willingness to travel up to 50% locally
Ability to work beyond standard hours as needed

Preferred Qualifications

Case management and discharge planning experience
Managed Care experience
Medicaid experience

Travel & Work Environment

Field-based role serving members in homes and community settings
Up to 50% regional travel -Counties Preferred: Ashland, Richland, Wayne, Fayette, Highland, Ross, Pike, Jackson, Licking, Muskingum, Coshocton, Fairfield, Franklin, Morrow, Washington
Occasional required travel to New Albany corporate office
Mileage reimbursement provided
