Full Job Description

Registered Nurse (RN) – Part-Time (CONTRACT)

Department: Clinical Services
Reports To: Program Director/Administrator
Employment Status: Part-Time (Flexible Schedule)
Location: Rel's Place LLC – Behavioral Health Residential Facility

Position Summary

The Registered Nurse (RN) is responsible for overseeing all medication-related services for youth residing at Rel's Place LLC. The RN ensures safe medication administration practices, maintains compliance with Ohio regulations and CARF standards, provides medication administration training (Medication Administration Record/MAR Training) to direct care staff, and serves as the clinical resource regarding medication management.

The RN works collaboratively with psychiatrists, primary care providers, pharmacies, and the interdisciplinary treatment team to promote the health and safety of each resident.

Essential Duties and ResponsibilitiesMedication Management

Oversee all medication administration processes within the facility.
Review and audit Medication Administration Records (MARs) for accuracy and completeness.
Ensure medications are administered according to physician orders.
Verify new medication orders and discontinue medications as ordered.
Monitor medication effectiveness and report concerns to prescribing providers.
Ensure medications are stored, secured, and disposed of according to state regulations.
Conduct routine medication cart and medication room inspections.
Monitor medication expiration dates.

Medication Counts

Perform routine medication counts.
Audit controlled substances and narcotic counts.
Investigate and document medication discrepancies immediately.
Ensure shift-to-shift medication counts are completed accurately.
Maintain documentation of medication inventories.

Staff Training

The Registered Nurse is responsible for training and competency validation of all employees authorized to administer medications.

Training responsibilities include:

Medication Administration Record (MAR) Training
Safe medication administration practices
The "Five Rights" of Medication Administration
Medication documentation requirements
Infection prevention during medication administration
Medication error prevention
Reporting medication incidents
Proper medication storage and security
Controlled substance procedures
Medication refusal procedures
Side effects and adverse reactions
Emergency medication procedures

The RN shall:

Conduct initial medication administration training.
Complete competency evaluations.
Observe return demonstrations.
Maintain employee training records.
Provide annual refresher training.
Retrain staff when medication errors occur.

Clinical Oversight

Monitor residents for medication side effects.
Review resident health concerns.
Coordinate physician appointments as needed.
Communicate with psychiatrists, physicians, pharmacies, and guardians regarding medication issues.
Assist with health assessments when needed.
Review hospital discharge instructions and medication changes.

Documentation

Maintain accurate documentation including:

Medication Administration Records (MARs)
Medication count sheets
Medication error reports
Staff competency forms
Medication training records
Nursing notes
Physician orders
Pharmacy communications

Compliance

Ensure compliance with:

Ohio Department of Mental Health and Addiction Services (OhioMHAS)
Ohio Administrative Code
CARF Accreditation Standards
HIPAA Confidentiality Requirements
OSHA Infection Control Standards
Rel's Place LLC Policies and Procedures

Quality Assurance

Participate in Quality Improvement activities.
Conduct monthly medication audits.
Identify trends in medication errors.
Recommend corrective actions.
Assist during CARF surveys and state inspections.
Maintain readiness for licensing reviews.

Emergency Responsibilities

Respond to medical emergencies involving residents.
Provide nursing consultation during health-related incidents.
Review incident reports involving medications.
Provide recommendations following medication-related events.

QualificationsRequired

Current Ohio Registered Nurse (RN) License in good standing.
CPR Certification.
Current First Aid Certification.
Knowledge of psychiatric medications.
Knowledge of child and adolescent behavioral health.
Experience with medication administration.
Excellent communication and organizational skills.
Ability to maintain confidential information.

Preferred

Minimum of one (1) year nursing experience.
Experience in behavioral health.
Experience working with children and adolescents.
Experience in residential treatment.
Experience with CARF standards.
Experience with OhioMHAS regulations.

Physical Requirements

Ability to lift up to 25 pounds.
Ability to sit, stand, bend, and walk throughout the facility.
Ability to respond quickly during emergencies.
Visual ability to accurately review medication labels and documentation.

Pay: $40.07 - $45.46 per hour
