Case Manager – Substance Use and Mental Health Treatment

Primary Job Responsibilities

Manage a weekly caseload of approximately 20–30 clients.
Meet with clients regularly to identify case-management needs, establish goals and monitor progress.
Complete case-management assessments, service plans and required updates.
Coordinate community resources, including housing, transportation, employment, legal, medical, financial and recovery-support services.
Assist clients with short-term disability, FMLA and other employer-related paperwork and communications, when applicable.
Advocate for clients and communicate with employers, courts, probation officers, children services and other community agencies as authorized.
Assist clients with aftercare and discharge planning to support continuity of care.
Facilitate case-management, life-skills, recovery-support and psychoeducational groups.
Maintain accurate, timely and thorough documentation in Kipu in accordance with company, payer, state and accreditation requirements.
Collaborate with therapists, medical providers, utilization review, behavioral health technicians and other members of the multidisciplinary treatment team.
Participate in treatment-team meetings, supervision, training and case consultation.
Help clients access benefits, identification documents, insurance, medication assistance and other necessary resources.
Follow up on referrals and document outcomes to ensure client needs are addressed.
Maintain professional boundaries and protect client confidentiality in accordance with HIPAA, 42 CFR Part 2 and all applicable laws and regulations.
Abide by Seacrest Recovery Center’s policies, code of ethics and organizational values.
Perform other case-management duties as assigned based on client and program needs.

Qualifications

Passion for working with individuals recovering from substance use and co-occurring mental health disorders.
Prior experience in behavioral health, substance use treatment, social services or case management preferred.
Strong client-advocacy, organization, follow-through and problem-solving skills.
Ability to manage a weekly caseload of approximately 20–30 clients while meeting documentation deadlines.
Comfortable facilitating groups and communicating with clients, families, employers and community agencies.
Ability to work collaboratively as part of a multidisciplinary treatment team.
Strong written and verbal communication skills.
Computer proficiency, including Microsoft Word, Excel and email. Experience with Kipu or another electronic health record system is a plus.
CDCA or other applicable behavioral-health credential preferred.
Associate or bachelor’s degree in social work, psychology, behavioral health, human services or a related field preferred.

Job Type: Full-time

Pay: $20.00 - $22.00 per hour
