Full Job Description

Job Overview

We are seeking a dynamic and compassionate Family Medicine Outpatient Nurse Practitioner to join our dedicated healthcare team. In this vital role, you will provide comprehensive primary care services to diverse patient populations in an outpatient setting. Your expertise will help promote health, prevent illness, and manage chronic conditions through patient-centered care and collaborative practice. Duties

Conduct thorough patient assessments, including physical examinations, vital signs, and medical history reviews to determine health status and needs.
Prescribe medications, administer injections, following HIPAA regulations and clinical guidelines.
Manage acute and chronic illnesses by developing personalized treatment plans, monitoring progress, and adjusting therapies accordingly.
Collaborate with physicians, specialists, and other healthcare professionals to coordinate patient care across multiple disciplines such as gastroenterology, geriatrics, behavioral health, and emergency medicine.
Utilize EMR/EHR systems for accurate documentation of medical records, coding (ICD-10/CPT), and billing processes.
Provide patient education on disease prevention, medication management, nutrition, and lifestyle modifications to foster health coaching and self-care.
Participate in clinical research initiatives or quality improvement projects to enhance patient outcomes and practice standards.

Requirements

Valid Nurse Practitioner license with current certification in family medicine or primary care.
Proven experience working in outpatient clinics with a focus on primary care services.
Proficiency in EMR/EHR systems for medical documentation and coding.
Strong knowledge of physiology, pharmacology, medical terminology, and diagnostic evaluation techniques.
Experience with diverse patient populations including youth, adults, mental health, and medication assisted treatment for addiction.
Excellent client communication skills with a focus on patient assessment and education; ability to collaborate effectively within multidisciplinary teams.
