Position Summary:
The I: DD Program Specialist supports individuals with Intellectual and Developmental Disabilities (IDD) by coordinating, monitoring, and optimizing service delivery through a person-centered care approach. This role oversees participant services, ensures compliance with Connecticut state regulations and organizational policies, supervises Direct Support Staff, and collaborates with interdisciplinary teams to improve participant outcomes and community integration.

Position Scope:
The IDD Program Specialist manages participant services, oversees care coordination activities, supports staff development, maintains regulatory compliance, and contributes to program quality initiatives. The position requires regular communication with participants, caregivers, families, providers, and internal departments.

Key Responsibilities:

Develop, implement, and update ISPs using person-centered planning aligned with participant goals, clinical needs, and program standards.

Manage caseloads, conduct assessments and home visits, and coordinate services while advocating for participant access to community and healthcare resources.

Collaborate with interdisciplinary teams and participate in ISP meetings to support effective service delivery and community integration.

Train and support caregivers and direct support staff on care plans, policies, and compliance requirements.

Maintain accurate, timely documentation in compliance with Connecticut regulations, Medicaid requirements, and organizational standards.

Monitor service quality and outcomes, supporting audits, compliance reviews, and quality improvement initiatives.

Assist with onboarding, training, and organizational projects as needed.

Regulatory Frameworks:

Connecticut state regulations for IDD and waiver services
Medicaid requirements
HIPAA privacy and confidentiality standards
Federal and state labor and healthcare regulations

Confidentiality:
Must maintain strict confidentiality of participant, employee, and organizational information in accordance with HIPAA, company policy, and applicable federal and Connecticut privacy laws.

Documentation Requirements:
Required documentation includes but is not limited to:

Progress notes
Incident reports
Assessments and evaluations
ISP documentation and updates
Home visit records
Training records and compliance documentation

Documentation must be accurate, complete, and submitted within required timelines.

Certifications:
Preferred or required certifications may include:

CPR/First Aid Certification (preferred)
Additional training or certifications as required by Connecticut regulations or organizational policy.

Education:
Bachelor’s degree in Social Work, Psychology, Special Education, Human Services, or a related field preferred.

Experience:
Minimum 2 years of experience in IDD services, case management, behavioral health, human services, or a related role preferred.

Experience working with individuals with Intellectual and Developmental Disabilities strongly preferred.

Technical Skills:

Electronic health record (EHR) and case management systems proficiency.
Microsoft Office Suite proficiency.
Strong documentation and reporting abilities.
Ability to use virtual communication and scheduling tools.

Languages:
Strong verbal and written English communication skills required. Bilingual abilities may be preferred depending on participant population and business needs.

Driver License:
Valid Connecticut Driver’s License ,reliable transportation and acceptable driving record may be required.

Travel Requirement:
Local travel may be required for home visits, participant meetings, trainings, and community activities.
