Qualifications:
- DDS/DMD degree from US accredited dental school or residency program

Active Florida dental license

Active Federal DEA and applicable certifications
- 5 or more years applicable professional clinical experience preferred

Advanced Dental Education Program recent graduates with Specialty Residency in Periodontics encouraged to apply
