We are a locally owned home healthcare company dedicated to helping individuals maintain their independence, dignity, and quality of life in the comfort of their own homes. We are seeking compassionate, dependable, and experienced Physical Therapists (PTs) and Occupational Therapists (OTs) who are passionate about providing exceptional, patient-centered care.
Position Summary
As a Home Health Physical Therapist or Occupational Therapist, you will provide individualized therapy services to patients in their homes. You will evaluate patients, develop and implement treatment plans, monitor progress, and help patients achieve greater independence, safety, mobility, and quality of life.
This role is ideal for therapists who value meaningful one-on-one patient care and enjoy working independently while being supported by a collaborative home health team.
Responsibilities
- Perform comprehensive physical or occupational therapy evaluations in the patient's home.
- Develop individualized treatment plans based on patient needs, goals, and physician orders.
- Provide skilled PT or OT interventions to improve strength, mobility, balance, coordination, function, and independence.
- Educate patients and caregivers on exercises, home programs, safety, and strategies for maximizing independence.
- Assess fall risks and recommend appropriate safety and mobility strategies.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Communicate effectively with physicians, nurses, caregivers, families, and other members of the care team.
- Complete accurate and timely clinical documentation in accordance with company and regulatory requirements.
- Follow all applicable patient care, safety, infection control, and professional standards.
- Promote patient dignity, independence, and quality of life throughout the course of care
Qualifications
- Current Iowa license as a Physical Therapist (PT) or Occupational Therapist (OT).
- Graduate of an accredited PT or OT program.
- Previous home health experience preferred but not required.
- Valid driver's license and reliable transportation.
- Strong clinical, communication, organizational, and interpersonal skills.
- Ability to work independently and manage a flexible schedule.
- Compassionate, dependable, and professional demeanor.
- Commitment to providing high-quality, patient-centered care.
Preferred Qualifications
- Previous experience in home healthcare, rehabilitation, skilled nursing, or outpatient/inpatient therapy.
- Familiarity with Medicare home health documentation and requirements.
- Experience working with older adults and patients with complex medical or mobility needs.
Scheduling Requirements
- Flexible scheduling available.
- Ability to travel to patients' homes throughout Iowa City and surrounding communities.
- Must be willing to commit to working at least one weekend per month.
- Various scheduling options may be available based on patient needs and therapist availability.
