Full Job Description

Our organization is seeking exceptional individuals who are eager and willing to work as a full-time Case Manager. The ideal candidate will perform case management services and will assess, plan, implement and monitor treatment plans for patients and families in need of assistance in our Residential Treatment Center. The focus of this position is to help beneficiaries get the best treatment available to them. This is a hands on position and requires on call hours. The position also requires you to work in a direct support capacity should there be an last minute need for coverage. Minimum 2 years experience. Bachelor degree required

Responsibilities Include:

To establish and build supportive and nurturing relationships with the youth.
Schedule and facilitate medical appointments
Update documentation for plan of service meetings
To coordinate all services to meet the youth’s needs and
Act as a liaison between youth, staff, managing conservator, school and all service providers.
Participate in youth intake and assessment interviews
Maintain youth files and develop goal-oriented service plans
Maintain confidentiality for all youth and program operations
Provide and maintain appropriate documentation
Establish helping relationships with youths and families/guardians
Provide advocacy as specified by Special Parent Advocacy Group and contract guidelines
Facilitate and participate in individual and group counseling
Prepare and submit documentation, assessments and reports within required timelines
Provide crisis intervention for youth and families
Attend and participate in training and workshops
Maintain professional and ethical standards
Comply with both the agency and program policy and procedure manuals
Comply with all Licensing Standards as well as funding source guidelines
Training will be required in Little Elm until Sanger location is open

Job Type: Full-time

Pay: $50,000.00 - $55,000.00 per year
