MEDICAL SOCIAL WORKER (MSW) | Case Management - $2,780.00/wk (13W -Days) | Mission Viejo, CA!

Providence Mission Hospital is seeking an experienced Medical Social Worker (MSW) to join its Acute Care Case Management team. Candidates must have recent acute care inpatient hospital case management/discharge planning experience. Qualified profiles are expected to move quickly!

POSITION DETAILS:

Title: Medical Social Worker (MSW)
Facility: Providence Mission Hospital
Location: Mission Viejo, CA
Specialty: Acute Care Case Management / Discharge Planning
Department: Care Management
Duration: 13 weeks (possible extn.)
Shift: Days | 5x8 Hours (8:00 AM – 4:30 PM)
Guaranteed Hrs: 40 Hours/Week
Start Date: 08/31/2026 & Later
Floating: May float to Laguna facility as needed

MINIMUM QUALIFICATION:

Master's Degree in Social Work (MSW)
Minimum 1 full year of recent Acute Care Inpatient Hospital Social Work / Case Management experience
Recent hospital experience within the last 3 months
Previous Travel Assignment experience required (No First-Time Travelers)
Experience with Epic EMR (Required – Hard Stop)

KEY RESPONSIBILITIES:

Complete psychosocial assessments within required timelines.
Develop individualized plans of care for hospitalized patients.
Coordinate post-acute services including home health, DME, rehabilitation, and skilled nursing placement.
Collaborate with physicians, nursing staff, utilization review, insurance case managers, and community agencies.
Maintain timely documentation in Epic EMR.
Manage an average caseload of 20–23 adult patients.

PREFERRED QUALIFICATION:

LCSW - preferred
BLS Certification
Trauma Level I or II Hospital experience
Experience with: Care Coordination | Utilization Review | Discharge Planning | Prior Authorizations | DME Coordination | Community Resource Navigation | Medicare & Medicaid Programs | Adult Protective Services | Advance Directives

COMPENSATION:

Local W2: $63.50/hr on W2 = $2,540.00/WK.
Travel W2: $25.00/hr on W2 + Stipends (Meals: $70/day | Housing: $150/day) = $2,780.00/WK.
