Position Summary:

Corvallis Family Medicine, a private practice group, situated in Corvallis Oregon, in the beautiful Willamette Valley, is seeking a dedicated, highly motivated provider to join our primary care team. This role would work alongside our dedicated physicians and APPs facilitating advanced primary and specialty services to our patients. Candidates are required to hold an Oregon Nurse Practitioner or Physician Assistant license. This FTE position is a 4 day per week (32 contact hours) schedule with one (phone triage only) on-call night approximately every 2 weeks and one weekend approximately every 8-9 weeks allowing abundant time to enjoy all this wonderful area offers. Our “call” is telephone triage only, as we have coverage for all hospital patients and admissions, can be done wherever you have phone and internet service. As a private clinic, we have larger latitude for our providers in scheduling, patient panel diversity, and patient volumes. The focus of this position would be family medicine with an emphasis on women’s health. CFM, a well-established (30+years) physician-owned, highly regarded, collaborative family medicine practice is offering an opportunity to follow your passion. Our single-site facility boasts in-house laboratory testing, EKG, minor procedure rooms for laceration repairs, dermatologic procedures, fracture management, a new Paxis 100 digital Straight Arm X-ray machine, as well as a handheld ultrasound machine to expedite our patients’ care as well as an EMR (eCW) in the cloud that boasts AI dictation capabilities. Our patients have access to behavioral health providers as part of our practice offerings. Many of our physicians and APPs have developed subspecialties to augment services available to our patients, including migraine and eating disorder treatment, osteomanipulation, infectious disease management, and women’s care. Our eight (8) medical provider (6 physicians and 2 APPS) and three (3) behavioral health providers (psychiatrist, social worker, and Licensed Professional Counselor) allows our providers scheduling flexibility to accommodate life’s ever-changing demands. We are close to world-class research/resource facilities, OHSU & the College of Osteopathic Medicine of the Pacific-Northwest (COMP-Northwest), where we have a strong history of training the next generation of physicians, and APPs and work collaboratively with their medical staff.

Essential Functions:
Delivery of Medical Care & Patient Centered Primary Care Home (PCPCH) examine, diagnose, and treat primary care and preventive health issues in an ambulatory setting, and provide counseling and education to assist patients in self-care. Prescribe and dispense medications appropriate to address medical conditions. Review lab results and take appropriate action. Perform in office medical procedures within the scope of practice and as defined by the practice protocol. Initiate and continue medical treatment plans for patients; accurately code and document in the EHR, complete documentation within the established time frame, and review reports to proactively manage care at the patient and panel level.
Contribute to develop and follow best practice standards for chronic disease management, health maintenance, tests and procedures, lab preference lists, and medication formulary.
Requirements
You may be a qualified PA candidate if you: have graduated from a PA program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or its predecessors, e.g., CAHEA, CAAHEP, or have passed the Physician Assistant National Certifying Examination (PANCE) and are certified by the National Commission on Certification of Physician Assistants (NCCPA), or a current PA license in good standing issued by the Oregon Medical Board (OMB), with a least 1 year of experience, hold DEA certification, and a current American Heart Association BLS certification. You may be a qualified NP candidate if you: possess a Master’s Degree or higher in Nursing, or in a field deemed to be equivalent by the Oregon Board of Nursing, or have satisfactorily completed a Nurse Practitioner Program recognized by the Oregon Board of Nursing, or hold a current, unencumbered Certificate as a Nurse Practitioner through the Oregon State Board of Nursing, or hold a valid unencumbered NP national certification issued by a nationally accrediting body, hold DEA certification, have at least 12-months relevant NP experience in primary care, and hold current American Heart Association BLS certification.
