Full Job Description

The Admissions Coordinator / Patient Access Manager at Serenity Oaks IRF manages the admissions intake process, census development, insurance verification, and pre-authorization activities to ensure appropriate, timely, and compliant patient admissions to the rehabilitation program. This role leads the admissions team, coordinates with clinical staff and referring providers on admission criteria, and serves as a key driver of census growth and revenue cycle integrity. The Admissions Manager ensures all admissions meet CMS 60% Rule criteria, medical necessity standards, and CARF admission policy requirements.

2. ESSENTIAL FUNCTIONS & RESPONSIBILITIES

Manage and coordinate the complete admissions intake process for all IRF referrals, including clinical screening, payer verification, and admission scheduling. Supervise Admissions Liaisons, Clinical Liaisons, and Authorization Specialists; manage team performance and workflows. Review incoming referrals for clinical appropriateness and IRF medical necessity in collaboration with the Medical Director. Ensure all admissions comply with CMS 60% Rule criteria; track 60% Rule compliance data and report to administration. Coordinate insurance pre-authorization for all admissions; verify benefits and obtain required authorizations prior to admission. Serve as the primary contact for referring hospitals, physician offices, and discharge planners regarding the admissions process. Develop and maintain referral relationships with area hospitals, physicians, and healthcare systems. Track and analyze admissions data including referral source, payer mix, conversion rates, and census trends. Collaborate with the IDT, Medical Director, and DON on admission appropriateness determinations. Ensure completion of all admission paperwork, consent forms, and patient rights notifications. Manage the admissions scheduling calendar to optimize bed utilization. Participate in CARF program evaluation activities related to admissions and patient access. Prepare census and admissions reports for leadership review.

3. EDUCATION & EXPERIENCE REQUIREMENTS

Required: Associate degree in healthcare administration, nursing, or related field; bachelor's degree preferred. Preferred: BSN or bachelor's in health administration with IRF or acute care admissions experience. Required: Minimum two (2) years of healthcare admissions, patient access, or utilization management experience. Preferred: IRF or post-acute admissions management experience.

4. LICENSURE, CERTIFICATION & REGISTRATION REQUIREMENTS

Preferred: Current RN license from the Louisiana State Board of Nursing (LSBN) for clinical admissions screening. Preferred: Certified Case Manager (CCM) or Certified Rehabilitation Counselor (CRC). Any held Louisiana clinical license must be verified prior to and throughout employment.

5. KNOWLEDGE, SKILLS & ABILITIES

Knowledge of IRF admission criteria, CMS 60% Rule, and medical necessity documentation requirements. Understanding of Medicare Part A, Medicaid, and managed care authorization processes. Strong clinical screening skills for IRF candidacy assessment. Leadership and supervisory skills for admissions team management. Proficiency with EHR admissions workflows and revenue cycle systems. Excellent communication and relationship-building skills with referring providers. Knowledge of CARF admissions standards and patient rights requirements. Ability to analyze admissions data and identify opportunities for census growth.

6. PHYSICAL DEMANDS & WORKING CONDITIONS

Combination of office and occasional patient room or referring facility environments. Primarily sedentary during documentation and authorization activities. Occasional travel to referring hospitals and physician offices. Occasional lifting up to 20 lbs.

Participate in CARF program evaluation related to admissions.
