About the Role

A single-location dermatology practice in Mandisonville, Louisiana. We are hiring a Revenue Cycle Specialist to serve as the sole owner of our billing function — the person responsible for ensuring every dollar of clinical work performed is captured, billed, collected, and reconciled.

This is not a claims-processing role. The right candidate understands that in a private practice, the biller is the tip of the spear for revenue. You will work directly with the practice owner and practice manager, with full visibility into financial performance and a meaningful voice in how we protect and grow it.

What Makes This Role Different

You will own the entire revenue cycle — not a queue inside it. That means:

You see revenue leaks before anyone else does, and you are expected to surface them.
You are the institutional memory for payer behavior, denial patterns, and reimbursement trends.
You are accountable for outcomes (days in A/R, clean claim rate, net collection rate) — not just tasks completed.
You partner with the practice owner on credentialing, payer contracts, and fee schedule reviews.

Core Responsibilities

Claims & Cash Posting

Submit clean claims to commercial, Medicare, Medicaid, and secondary payers within 48 hours of charge entry.
Post payments, adjustments, and denials from EOBs and ERAs daily; reconcile to deposits.
Work denial queues and appeals within payer timely-filing windows; track root causes.
Verify patient eligibility and benefits in advance of visits; flag coverage issues to the front desk.

Accounts Receivable Ownership

Manage the full A/R aging report; prioritize follow-up by payer, age bucket, and dollar value.
Run patient balance workflows: statements, payment plans, and pre-collection outreach.
Escalate problem accounts to the practice manager with a recommended action, not just a status.

Credentialing & Payer Enrollment

Maintain and renew CAQH profiles for all providers; never let an attestation lapse.
Manage initial credentialing and re-credentialing with all contracted payers.
Track provider enrollment effective dates and ensure no clinical work is performed under an unenrolled provider.
Maintain a credentialing tracker (payer, status, expiration, next action) accessible to the practice owner.

Financial Reconciliation

Perform monthly bank reconciliation against deposits posted in the practice management system.
Reconcile merchant services (card processing) deposits and identify variances.
Partner with the practice manager on month-end close: outstanding A/R, unposted payments, and credit balance review.

Revenue Integrity & Leak Identification

Proactively identify under-coded encounters, missed charges, and modifier opportunities — and raise them.
Monitor payer reimbursement against contracted rates; flag underpayments and pursue recovery.
Track denial trends by payer, CPT, and root cause; recommend front-end or clinical workflow fixes.
Review fee schedule annually against Medicare and commercial benchmarks; recommend adjustments.

Reporting & Accountability

Deliver a monthly revenue cycle dashboard to the practice owner covering: gross/net collections, days in A/R, A/R aging by bucket, clean claim rate, denial rate, and top denial reasons.
Maintain written documentation of billing workflows, payer-specific quirks, and credentialing status so the practice is never single-point-of-failure dependent on memory.
Flag material issues to the practice owner the same day they are identified — not in the monthly report.

Required Qualifications

3+ years of medical billing experience in a physician practice setting (specialty preferred over hospital or large-group experience).
Working knowledge of CPT, ICD-10, HCPCS, and modifier application — particularly for E/M, procedures, and pathology.
Hands-on experience with EHR/practice management software; comfort moving between PM, clearinghouse, and payer portals.
Demonstrated experience working denials and appeals to resolution — not just routing them.
Strong working knowledge of Medicare, Medicaid (Louisiana), and major commercial payer rules.
Ability to work independently, prioritize across competing demands, and own outcomes without daily direction.
High attention to detail; comfortable being the person who catches what others miss.

Strongly Preferred

Dermatology billing experience, including Mohs, pathology, biopsies, destructions, and cosmetic/medical split-billing considerations.
ModMed (EMA) experience.
CPC, CPB, CPMA, or CRCR certification.
Prior experience as the sole or lead biller in a private practice.
Experience with CAQH maintenance and payer credentialing workflows.

What Success Looks Like in Year One

Days in A/R reduced and held within specialty benchmark.
Clean claim rate consistently above 95%.
Zero lapsed CAQH attestations or expired credentials.
Monthly close is on time, every time, with reconciled deposits and a current dashboard.
At least three revenue improvement opportunities identified, documented, and implemented.
