ADMISSIONS COORDINATOR - SKILLED NURSING

POSITION SUMMARY

The main focus of the Admissions Coordinator is to assist in handling referrals to The Legacy in a way that satisfies occupancy and payor mix. This position will also need to become knowledgeable of other services that are offered by The Legacy and promote those services as opportunities arise.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

Performs on-site and telephone pre-admission screening and clinical assessments. Assists with pre-certification by providing a plan of care for managed care referrals and participates in external payor negotiations for approval of patient.

Communicates with members of referred patient’s social/support base as needed. Communicates with other members of the current interdisciplinary team, establishes preliminary care plan/discharge plan and communicates to admission/social work for consideration.

Greet and tour prospective residents and their families.

Complete all admission paperwork prior to admission and in a timely manner.

Notify appropriately to community departments when a resident is admitted, transferred, discharged or moved from one room to another.

Obtain required signatures on all admission forms and documents.

Supports and protects the interests of the facility at all levels - financial, professional and legal. Maintains high ethical standards. Demonstrates a thorough understanding of the acute hospital and sub-acute process related to cost-effective, quality care of each patient as an individual.

Maintains key relationship and partnerships with physicians, case managers, managed care insurance networks, social workers, area sub-acute and acute hospital sources. Maintains a thorough knowledge of facilities, products, services and acuity trends. Develops market plans/strategies and activity reports.

Educates clients, family members of significant others, referral sources, hospital personnel, external payors, and external case manages as to skilled nursing and rehabilitation benefits and services.

QUALIFICATIONS

Educational Requirements and Experience:

Bachelor’s degree (preferred)

Minimum of 5 years diverse retirement industry background in the Dallas metroplex area

Market knowledge of area hospitals, sub-acute care, assisted living, and long term care providers
