CC Young, a vibrant senior living community and a non-profit leader in senior living and Health Care is seeking a Home Health LVN – Intake Coordinator to be responsible for supporting the clinical and administrative operations of the home health agency through effective scheduling, referral coordination, medical record management, and clinical review of incoming referral documentation.
This position serves as a liaison between intake, clinical leadership, field clinicians, physicians/providers, referral sources, patients, and other members of the interdisciplinary team. Reviews referral and clinical documentation within the scope of LVN practice, coordinates timely services, identifies missing or inconsistent information, and escalates matters requiring RN-level assessment or clinical judgment.
Essential Job Duties and Responsibilities
Referral Review & Intake Support
- Review incoming home health referrals for completeness, including physician orders, H&P, face-to-face documentation, discharge information, medication lists, diagnoses, and therapy recommendations.
- Identify missing, incomplete, or inconsistent documentation and coordinate with referral sources, physician offices, hospitals, SNFs, ALFs, and other providers to obtain required information.
- Enter and maintain accurate referral information in the EMR and track referrals through acceptance, scheduling, admission, or non-admission.
- Communicate referral status and outstanding documentation needs to intake and clinical leadership.
- Escalate referrals requiring RN-level clinical review or determination to the appropriate clinical leader.
Scheduling & Clinical Coordination
- Coordinate and assign Start of Care (SOC), Resumption of Care (ROC), recertification, routine nursing, therapy, and other ordered visits based on patient needs, clinician availability, geographic location, and staffing.
- Monitor schedules for unassigned, missed, overdue, declined, or rescheduled visits and assist with coverage as needed.
- Communicate schedule changes promptly to patients, caregivers, clinicians, and leadership.
- Coordinate with RNs, LVNs, PTs, PTAs, OTs, COTAs, SLPs, MSWs, HHAs, and other interdisciplinary team members.
- Notify clinical leadership of scheduling barriers that may affect patient safety, ordered frequency, timely initiation of care, or continuity of services.
Medical Records Management
- Maintain complete, accurate, organized, and confidential patient medical records in accordance with agency and regulatory requirements.
- Review records for missing documents, signatures, dates, orders, and other required information.
- Upload, scan, index, and route documentation within the EMR.
- Track and follow up on outstanding physician/provider orders, plans of care, face-to-face documentation, and other required records.
- Assist with medical record preparation for audits, surveys, quality reviews, payer requests, and other regulatory needs.
- Process authorized medical record requests in accordance with agency policy and HIPAA requirements.
Clinical & Administrative Support
- Review clinical information within the LVN scope of practice and communicate concerns, inconsistencies, or significant findings to the supervising RN or clinical leadership.
- Assist with tracking admissions, recertifications, resumptions of care, discharges, and other patient status changes.
- Coordinate with intake, scheduling, clinical operations, QA, billing, authorization, and other departments to support timely patient care.
- Assist with follow-up documentation, case conferences, quality improvement activities, staff meetings, and survey readiness.
- Perform other clinical and administrative support duties consistent with LVN scope of practice and agency needs.
Compliance & Quality Responsibilities
- Perform all duties in accordance with applicable federal and Texas requirements, payer requirements, accreditation standards, and agency policies.
- Maintain patient confidentiality and comply with HIPAA requirements.
- Practice within the legal scope of a Texas Licensed Vocational Nurse.
- Immediately escalate matters requiring RN assessment, comprehensive clinical judgment, or responsibilities outside the LVN scope of practice.
- Report potential compliance, documentation, patient safety, or quality concerns to appropriate leadership.
Perform other duties as assigned
Minimum Qualifications
- Current and unrestricted Texas LVN license.
- Graduate of an approved vocational nursing program.
- Current CPR/BLS certification, if required by agency policy.
- Home health, hospice, skilled nursing, physician office, hospital, or other healthcare experience preferred.
- Healthcare experience required; home health, hospice, SNF, hospital, or physician office experience preferred.
- Knowledge of medical terminology, diagnoses, medications, physician orders, and clinical documentation.
- Basic understanding of home health referral and admission processes.
- Strong computer and data-entry skills.
- Ability to maintain confidentiality and appropriately handle protected health information.
Preferred Qualifications
- Home health scheduling, intake, referral coordination, or medical records experience strongly preferred.
- One or more years of home health experience.
- Experience with WellSky/Kinnser, MatrixCare, or similar EMR systems.
- Familiarity with Medicare home health documentation, face-to-face requirements, plans of care, physician orders, audits, surveys, and payer requests.
- Experience preparing records for audits, surveys, QA review, or payer requests.
Knowledge, Skills & Abilities
- Strong organizational and time-management skills.
- Excellent attention to detail and follow-through.
- Ability to prioritize multiple referrals, schedules, and documentation deadlines.
- Strong written and verbal communication skills.
- Ability to recognize issues requiring RN or clinical leadership involvement.
- Ability to work independently and collaboratively in a fast-paced home health environment.
- Knowledge of HIPAA, confidentiality, and professional healthcare standards.
Physical Requirements
The employee must be able to perform the essential functions of the position with or without reasonable accommodation. The position primarily involves office and computer-based work and may require prolonged periods of sitting, telephone communication, computer use, filing, and handling medical records.
