Regional Hospice of Western CT has been dedicated to empowering patients with end-of-life care choices since 1983. We provide compassionate support in private homes, assisted living, skilled nursing facilities, and our Center for Comfort Care & Healing, focusing on creating meaningful connections during life’s most sacred transitions.

Summary

We are seeking a full time (40 hrs) medical Social Worker to join our team at Regional Hospice of Western CT. In this role, you will provide essential support to patients and their families, helping them navigate the complexities of end-of-life care while honoring their unique journeys. This position plays a vital role in enhancing the quality of life for those we serve. The territory we cover is Fairfield County into a piece of Litchfield and New Haven Counties.

Responsibilities

Conduct assessments to identify patient needs and develop individualized care plans.
Provide mental health counseling and support to patients and families during challenging times.
Facilitate group therapy sessions to promote emotional healing and connection.
Collaborate with interdisciplinary teams to ensure comprehensive patient care.
Utilize motivational interviewing techniques to empower patients in their care decisions.
Perform diagnostic evaluations to assess mental health conditions and recommend appropriate interventions.
Support patients with developmental disabilities by advocating for their needs within the healthcare system.

Requirements

Medical social worker with a master's degree in social work (MSW) from an accredited institution required.
Current licensure as a Social Worker in the state of Connecticut.
Experience in hospice or palliative care settings preferred.
Strong skills in mental health counseling and clinical assessment.
Ability to work effectively within a multidisciplinary team environment.
Excellent communication and interpersonal skills.
