Full Job Description

CoveCare Center has an FT Social Care Navigator position. You will approach the position with enthusiasm to develop positive client relationships, as one of our primary goals is to keep our clients happy and healthy. Responsibilities:

1. Develops rapport with clients (in person or on the telephone) in order to gather appropriate and correct information.

2. When the client arrives at CoveCare for the designated Open Access hours, outreach to Medicaid recipients to see if they can be screened for Social Care Navigation services

3. Monitors the status of cases until they are accepted by the referrant, through telephone and mail contact

4. Uses Medicaid recipient data from Netsmart, in agency programs, and lists given by HVCC to contact as many Medicaid recipients as possible via phone or in person

5. Completes screenings and provides navigation services for any Medicaid recipient who qualifies

6. Refers to crisis intervention, as necessary.

7. Return all calls within 24 business hours

8. 100% of information gathered will be entered into the EHR

9. Maintains monthly statistics

10. 100% of required trainings will be completed within specified time frames.

11. Undertakes related duties as assigned

Qualifications:

Experience: 3 years of related experience. Relevant experience would be considered in lieu of a degree, Others: A. Strong organizational and computer skills. B. Strong phone skills and ability to multi-task in a fast-paced environment. C. Proficiency in Microsoft office, specifically excel and Medical Record (EMR) preferred. D. Ability to maintain confidentiality and exercise good judgment in all client interactions

*** This is a full-time hybrid position with a Monday - Friday flexible schedule. Hours may change based on business needs and operations.
