Full Job Description

SUPPORTIVE HOUSING NAVIGATOR

Danielson, Connecticut

Full-Time: 35 Hours per Week

Monday–Friday, 8:00 a.m.–4:00 p.m.

$23.00–$24.00 per Hour

BACHELOR’S DEGREE REQUIRED

Applicants must have a bachelor’s degree in social work, human services, psychology, sociology, counseling, family studies, human development, or another closely related human services field.

Work experience cannot replace the required bachelor’s degree.

ABOUT THE POSITION

Access Community Action Agency is hiring a Supportive Housing Navigator to help families achieve stable housing, strengthen their independence, and connect with needed services.

This is a community-based position that includes frequent home visits and travel throughout Eastern Connecticut.

WHAT YOU WILL DO

Provide supportive, family-centered case management.
Maintain a caseload of approximately 12 families.
Meet with families in their homes and in the community.
Help families find and maintain safe, affordable housing.
Develop service plans based on each family’s needs and goals.
Help families build skills related to budgeting, parenting, household management, employment, and independent living.
Connect families with healthcare, behavioral health, recovery, childcare, education, employment, transportation, and public benefits.
Work with DCF, The Connection, Inc., landlords, schools, and community providers.
Coordinate and participate in Family Team Meetings.
Maintain clear and timely case notes and program records.

WHAT YOU NEED

Bachelor’s degree in a qualifying human services field.
At least one year of experience in case management, child welfare, supportive housing, social services, behavioral health, or a related human services setting.
Strong communication, organization, and problem-solving skills.
Ability to work independently and manage appointments, home visits, deadlines, and documentation.
Ability to work respectfully with families experiencing housing instability and other challenges.
Valid driver’s license and reliable transportation.
Ability to travel throughout Eastern Connecticut.

PREFERRED

Bilingual in English and Spanish.
Experience working with DCF-involved families.
Experience with supportive housing, family preservation, or community-based case management.
Knowledge of housing programs, public benefits, and Eastern Connecticut community resources.
