Dedicated Clinical & Administrative Support
Each physician practices within a collaborative care team ("Pod") supported by a dedicated Regional Nurse Coordinator who serves as an extension of the physician's clinical team. Depending on the physician's practice, care may be provided across Skilled Nursing Facilities, Assisted Living Communities, Inpatient Detoxification & Recovery Programs, and Correctional Healthcare settings.
Regional Nurse Coordinators assist with:
- Daily scheduling and care coordination
- Documentation and note management
- Facility communication
- Prescription refills and medication-related requests
- Orders, forms, and administrative follow-up
- Communication with nursing staff, families, pharmacies, specialists, behavioral health teams, correctional staff, and other healthcare partners
This team-based model significantly reduces administrative burden, improves communication, and allows physicians to remain focused on clinical decision-making and exceptional patient care.

Position Responsibilities
The physician will provide comprehensive medical care across one or more of our partner healthcare settings, including:
- Skilled Nursing Facilities
- Assisted Living Communities
- Independent Living Communities
- Inpatient Detoxification & Recovery Programs
- Correctional Healthcare Facilities
Responsibilities include:
- Performing comprehensive medical evaluations
- Diagnosing and managing acute and chronic medical conditions
- Providing admission, follow-up, and discharge evaluations, as appropriate
- Developing individualized treatment plans
- Managing patients with complex medical, psychiatric, and substance use disorders
- Collaborating with Advanced Practice Providers, nursing staff, behavioral health professionals, specialists, correctional staff, facility leadership, families, and healthcare partners
- Participating in interdisciplinary care planning
- Providing medical leadership within a collaborative care team
- Delivering evidence-based, patient-centered medical care
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Active Pennsylvania Medical License (or eligibility to obtain)
- Current DEA registration or ability to obtain
- Strong clinical judgment and communication skills
- Ability to practice independently while collaborating within a multidisciplinary team
- Commitment to providing compassionate, patient-centered care
Experience in Long-Term Care, Geriatrics, Family Medicine, Internal Medicine, Hospital Medicine, Addiction Medicine, Correctional Medicine, Emergency Medicine, or Post-Acute Care is welcomed but not required.
