Overview
Join our dedicated team as a Long-term Care Coordinator in an office-based role, where your organizational skills and compassionate approach will make a meaningful difference in the lives of individuals requiring ongoing care. In this vital position, you will serve as a central point of contact, coordinating services, managing provider schedules, prepping clinical charts and ensuring seamless communication among care providers, clients, and our clients. Your proactive engagement and attention to detail will help foster a supportive environment that promotes well-being and independence for those we serve.

Duties

Act as the primary liaison between clients, healthcare providers.
Schedule and facilitate client visits, prepping charts, and follow-up visits to ensure timely delivery of services.
Maintain accurate and up-to-date client records, including care plans, progress notes, and service documentation in compliance with privacy regulations.
Provide guidance and support to clients regarding available services, eligibility criteria, and necessary paperwork.
Monitor client progress by tracking service delivery, addressing concerns promptly, and adjusting care plans as needed.
Collaborate with multidisciplinary teams to ensure coordinated efforts that meet each client's unique needs.
Prepare detailed reports on client status, service utilization, and care outcomes for internal review and external agencies.

Qualifications

Proven experience in healthcare coordination, or related administrative roles within long-term care settings.
Excellent organizational skills with the ability to manage multiple cases efficiently while maintaining attention to detail.
Strong communication skills to clearly convey information to clients, healthcare professionals, and team members.
Knowledge of healthcare regulations, privacy laws (such as HIPAA), and community resources related to long-term care.
Ability to handle sensitive situations with empathy and professionalism.
Proficiency in office software applications such as Healthcare EMRs, Pointclickcare or eClinicalworks
Nice-to-have skills include familiarity with electronic health record systems (EHR), basic knowledge of insurance processes, or certifications in case management or social services.
