Full Job Description

Job Overview

The Medical Records & Referral Coordinator is responsible for maintaining patient medical records, processing referral documentation, responding to record requests, and ensuring compliance through routine audits. This position also supports check-in and check-out including patient communication related to referrals and documentation needs.

Responsibilities

Create and maintain patient records
Ensure the accurate and complete documentation of patient information, including medical history, diagnoses, treatments, and test results
Retrieve patient records as needed for providers, staff, and authorized personnel
Ensure records are accessible in a timely manner to support patient care
Process requests for medical records from patients, legal representatives, insurance companies, and other authorized parties
Upload and process referrals and ensure appropriate routing to clinical staff/providers
Create new patient accounts and enter patient demographics
Receive phone calls from patients and providers with questions and concerns regarding referrals
Process audits to assess the accuracy and completeness of medical records
Responsible for verifying eligibility relating to patient insurance for primary, secondary and tertiary plans
Responsible for collection of patient balances including co-insurance, deductibles, and co-pays
Track missing documentation and follow up as needed with outside providers
Maintain confidentiality and compliance with HIPAA regulations at all times
Manage both paper and electronic medical charts, ensuring accurate filing and documentation.
Upload, scan, and index medical records into the Electronic Health Record (EHR) in a timely and organized manner.
Monitor incoming faxes and electronic documents, filter and route information to the appropriate department/provider.
Communicate professionally with internal teams and external provider offices

Requirements

High school diploma or GED required (medical office training or certification preferred)
1+ year experience in a medical office setting preferred
Experience with medical records, referrals, or insurance processes strongly preferred
Knowledge of HIPAA and patient privacy standards
Strong computer skills and knowledge of Nextgen EHR systems
Excellent attention to detail and organizational skills
Strong written and verbal communication skills
Ability to manage multiple tasks and deadlines in a fast-paced environment
Professional phone etiquette and customer service mindset

Preferred Skills

Experience in specialty practice (ophthalmology, surgical practices, etc.)
Familiarity with referral authorization processes
Experience handling record requests, release forms, and audit prep
Understanding of medical terminology

Pay: $16.00 - $19.00 per hour
