Full Job Description

Certified Professional Coder - Prospective Chart Review

We are seeking an experienced Certified Professional Coder (CPC) with extensive knowledge of clinical indicators, medical coding guidelines, and documentation requirements. This role is responsible for conducting prospective medical record reviews to identify documentation and coding opportunities before the visit, supporting accurate code assignment, regulatory compliance, and appropriate reimbursement.

Key Responsibilities

Conduct prospective chart reviews to identify and suspect all active MRA conditions to be addressed during the office visit.
Review clinical indicators, including diagnoses, symptoms, laboratory results, imaging, medications, treatment plans, and other relevant findings to suspect conditions based on clinical indicators.
Identify documentation gaps, conflicting information, and potential coding or compliance concerns.
Validate diagnosis and procedure code assignment using current ICD-10-CM, CPT, and HCPCS guidelines.
Ensure coded conditions are supported by documented clinical evidence and meet applicable reporting requirements.
Collaborate with coding, clinical documentation integrity, compliance, quality, and revenue cycle teams.
Monitor coding and documentation trends and communicate findings to appropriate stakeholders.
Maintain productivity and accuracy standards while protecting patient confidentiality.
Remain current on regulatory updates, official coding guidelines, payer requirements, and industry best practices.

Required Qualifications

Current Certified Professional Coder (CPC) credential through AAPC.
Minimum of five years of professional coding or chart-review experience, including prospective and suspect chart reviews.
Extensive knowledge of clinical indicators and their relationship to potential new suspects and documented diagnoses and coded conditions.
Advanced knowledge of ICD-10-CM, CPT, HCPCS, medical terminology, anatomy, physiology, pharmacology, and disease processes.
Strong understanding of official coding guidelines, payer policies, medical necessity, and compliant query practices.
Demonstrated ability to interpret complex medical records and communicate documentation concerns clearly and professionally.
Strong analytical, organizational, and written communication skills.
Experience working with electronic health records and coding or encoder software.
Ability to work independently, prioritize competing assignments, and meet quality and productivity expectations.

Preferred Qualifications

Additional credentials such as CCS, CRC, COC, CPMA
Experience with risk-adjustment, hierarchical condition categories, clinical documentation integrity, or value-based care programs.
Experience reviewing records across multiple specialties and care settings.
Familiarity with healthcare auditing, compliance monitoring, and provider education.
Experience conducting remote chart reviews.

This individual will work collaboratively with providers and operational teams to strengthen documentation quality, improve coding accuracy, and reduce compliance and reimbursement risk.
