Full Job Description

Music Therapist – Hospice & Palliative Care

T.O.N.E. Hospice & Palliative Care, Inc. is seeking a compassionate and creative Music Therapist to provide therapeutic music services to hospice and palliative care patients and their families. The Music Therapist will work as part of our interdisciplinary care team to enhance comfort, emotional well-being, quality of life, and meaningful connection for patients facing serious or life-limiting illness.

Responsibilities

Provide individualized music therapy services to hospice and palliative care patients in homes, assisted living facilities, skilled nursing facilities, and other care settings.
Complete music therapy assessments and develop individualized interventions based on each patient's needs, preferences, abilities, and goals.
Use live and recorded music, singing, instrument playing, songwriting, relaxation techniques, life review, and other appropriate therapeutic interventions.
Support patients experiencing anxiety, restlessness, isolation, emotional distress, pain, or difficulty communicating.
Provide emotional support and meaningful engagement for patients and their families.
Modify interventions based on the patient's medical condition, cognitive status, cultural background, and personal music preferences.
Collaborate with nurses, physicians, social workers, chaplains, hospice aides, and other members of the interdisciplinary team.
Communicate significant changes or concerns regarding patients to the appropriate clinical team members.
Complete accurate and timely documentation of visits and patient responses.
Participate in interdisciplinary team meetings as required.
Maintain patient confidentiality and comply with HIPAA and agency policies.
Provide compassionate, respectful, patient-centered care at all times.

Qualifications

Bachelor's degree or higher in Music Therapy from an accredited program.
MT-BC (Music Therapist – Board Certified) credential preferred.
Previous experience in hospice, palliative care, geriatrics, home care, behavioral health, or a healthcare setting preferred.
Strong communication and interpersonal skills.
Ability to work independently in community-based settings while collaborating with an interdisciplinary team.
Compassionate and comfortable working with patients and families experiencing serious illness, end-of-life care, and grief.
Reliable transportation and ability to travel to patient locations.
Valid driver's license and current automobile insurance.
