Full-Time Hospice RN Case Manager

We are seeking an experienced Full-Time Hospice RN Case Manager who combines strong clinical judgment with compassion, organization, efficiency, and the ability to guide patients and families through complex and emotional journeys.

This position primarily serves patients throughout Oakland, Macomb, and Wayne counties.

What You’ll Do

Manage an assigned caseload of hospice patients in community, home, and facility settings
Complete comprehensive hospice nursing assessments and develop individualized plans of care
Perform admissions, start-of-care visits, recertifications, routine visits, PRN visits, and ongoing patient evaluations
Provide skilled nursing care focused on comfort, pain control, symptom management, and quality of life
Assess changes in patients’ conditions and communicate concerns promptly
Educate patients and families regarding disease progression, medications, symptom management, safety, and what to expect throughout the hospice journey
Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, facility staff, caregivers, and other interdisciplinary team members
Participate in interdisciplinary group meetings and contribute to updates to each patient’s plan of care
Provide emotional support, education, and guidance to patients and their families
Communicate with the care team in real time using Home Health Notify, our HIPAA-compliant communication platform
Complete clinical documentation and care planning using Careficient EMR
Ensure documentation is timely, accurate, complete, and compliant with Medicare, hospice, and agency requirements
Maintain communication with facilities, caregivers, physicians, and agency leadership
Participate in the agency’s on-call rotation as assigned
Travel throughout the assigned service area to meet patient-care needs

What We’re Looking For

Graduate of an accredited school of nursing
Active and unrestricted Michigan Registered Nurse license
Previous hospice nursing experience required
Hospice experience providing care in private homes, community settings, assisted living communities, skilled nursing facilities, or other facility settings is strongly preferred
Candidates with hospice experience in other clinical settings will also be considered
Minimum of two years of nursing experience preferred
Experience independently managing a hospice patient caseload preferred
Knowledge of hospice eligibility, symptom management, end-of-life care, plans of care, admissions, recertifications, and interdisciplinary care coordination
Strong clinical assessment, critical-thinking, and decision-making skills
Compassionate, calm, dependable, and patient-focused
Excellent communication, organization, and time-management skills
Ability to work independently in the field while collaborating closely with an interdisciplinary team
Comfortable using technology for communication and clinical documentation
Valid driver’s license, current automobile insurance, and reliable transportation
Ability to travel throughout Oakland, Macomb, and Wayne counties
