Full Job Description

Director of Nursing

Position Summary

We are seeking an experienced Director of Nursing to lead clinical compliance, quality assurance, accreditation readiness, and clinical operations for a growing home health agency.

This role is ideal for a home health leader with strong experience in CHAP accreditation, QAPI, Medicare Conditions of Participation, clinical audits, supervisory visits, and survey preparation. The Director of Nursing will be responsible for building and maintaining the clinical systems needed to support compliant, high-quality, and scalable home health operations. The position will initially support operations in Palm Beach County, and transition to full time DON role in Broward County over next 3-4 months.

The successful candidate should be highly organized, detail-oriented, and comfortable developing clinical processes rather than simply maintaining existing ones.

Key Responsibilities

Clinical Leadership

Provide overall clinical oversight of home health services.
Review patient referrals and determine clinical appropriateness for admission.
Oversee patient admissions, assessments, reassessments, and Plans of Care.
Ensure clinical services are delivered in accordance with physician orders, agency policies, payer requirements, and applicable regulations.
Provide clinical guidance and support to nursing staff.
Monitor patient outcomes, care coordination, and quality of care.
Address clinical concerns, incidents, complaints, and changes in patient condition.

CHAP Accreditation and Survey Readiness

Lead implementation and ongoing maintenance of CHAP standards.
Prepare the agency for initial and ongoing accreditation surveys.
Review and update policies, procedures, forms, and clinical workflows to ensure compliance.
Conduct internal mock surveys and readiness assessments.
Identify compliance deficiencies and implement corrective action plans.
Maintain continuous survey readiness rather than relying on last-minute preparation.

QAPI and Quality Assurance

Develop, implement, and manage the agency’s QAPI program.
Establish measurable quality indicators and performance-improvement initiatives.
Collect, analyze, and report clinical quality data.
Lead QAPI meetings and maintain required documentation.
Monitor trends involving hospitalizations, incidents, complaints, infections, documentation deficiencies, and patient outcomes.
Develop and oversee performance-improvement plans based on identified trends.

Clinical Compliance

Ensure compliance with Medicare Conditions of Participation, state home health regulations, accreditation standards, and payer requirements.
Conduct routine patient-chart audits and clinical documentation reviews.
Monitor completion of reassessments, supervisory visits, physician orders, Plans of Care, and recertification requirements.
Ensure clinical records are complete, accurate, timely, and survey-ready.
Oversee corrective actions related to documentation or clinical-performance deficiencies.
Maintain knowledge of regulatory changes and implement necessary updates.

Supervisory Visits and Reassessments

Complete or oversee required supervisory visits.
Complete or oversee periodic reassessments and recertifications.
Ensure Plans of Care accurately reflect patient needs and current clinical status.
Coordinate updates with physicians, families, field staff, and payers as needed.
Ensure all required visits and reviews are completed within applicable timeframes.

Staff Competency and Education

Participate in the recruitment and clinical screening of nursing staff.
Oversee clinical orientation and competency validation.
Develop and maintain annual competency and education requirements.
Provide education, coaching, and corrective guidance to clinical staff.
Ensure clinicians remain compliant with licensure, training, and competency requirements.
Support a culture of accountability, patient safety, and continuous improvement.

Clinical Systems Development

Develop scalable clinical workflows for admissions, recertifications, authorizations, supervisory visits, chart reviews, and incident management.
Create tracking systems for clinical deadlines and compliance requirements.
Collaborate with leadership and support staff to improve efficiency and reduce compliance risk.
Help establish standardized clinical processes that can support future organizational growth.

Qualifications

Active and unrestricted Registered Nurse license.
Prior Director of Nursing, Clinical Director, or comparable home health leadership experience.
Direct experience working in a Medicare-certified home health agency.
Demonstrated experience with CHAP accreditation and survey preparation.
Strong knowledge of Medicare Conditions of Participation and home health regulatory requirements.
Experience developing and managing a QAPI program.
Experience conducting chart audits, supervisory visits, reassessments, and clinical compliance reviews.
Strong understanding of Plans of Care, physician orders, recertification, and clinical documentation requirements.
Excellent organizational, leadership, communication, and problem-solving skills.
Ability to work independently and take ownership of clinical compliance.
Experience building or improving home health clinical systems is strongly preferred.
Experience with pediatric private duty nursing is preferred but not required.
Experience with home health EMR systems is preferred.

Ideal Candidate

The ideal candidate is a hands-on clinical compliance leader who understands both the regulatory and practical sides of home health operations.

This individual should be capable of independently building and managing CHAP compliance, QAPI, supervisory-visit processes, reassessments, clinical audits, staff competencies, and survey readiness. They should be comfortable identifying deficiencies, developing solutions, and ensuring that clinical requirements are consistently completed and documented.

This position is best suited for someone who enjoys creating structure, improving systems, and helping build a high-quality clinical organization.
