Full Job Description

Overview
We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will possess strong clinical skills and a commitment to providing high-quality patient care. As an LVN, you will play a vital role in supporting patients with various health needs, including those requiring emergency medicine, dementia care, and critical care. Your expertise in triage and vital signs monitoring will be essential in ensuring optimal patient outcomes.

Responsibilities

Provide direct patient care, including administering medications and treatments as prescribed.
Conduct assessments and monitor patients’ vital signs to identify any changes in their condition.
Assist with triage processes to prioritize patient needs effectively.
Collaborate with healthcare team members to develop and implement individualized care plans.
Support patients with activities of daily living and provide emotional support as needed.
Maintain accurate and timely documentation of patient care using electronic health record systems such as Athenahealth.
Participate in sterile processing procedures to ensure a safe environment for patients.
Educate patients and their families on health management practices, including occupational health strategies.

Requirements

Current Licensed Vocational Nurse (LVN) license in the state of practice.
Strong clinical skills in emergency medicine, critical care, and dementia care preferred.
Proficiency in monitoring vital signs and performing triage assessments.
Familiarity with electronic health record systems, particularly Athenahealth, is a plus.
Excellent communication skills and the ability to work collaboratively within a healthcare team.
Compassionate demeanor with a genuine interest in providing quality patient care.
Ability to handle stressful situations calmly and effectively.
