Full Job Description

Adolescent Care Navigator

This position is a high-touch client services role that provides care coordination services and early intervention to adolescents with co-occurring substance abuse and mental health disorders in the Denver Metro-area. Adolescent Care Navigators will work closely with internal and external therapists, case managers, other medical professionals, probation officers, DHS case workers, and other key client facing roles. The Adolescent Care Navigator will also be responsible for establishing and maintaining community and collateral connections to foster referrals and strengthen the safety net for youth.

Adolescent Care Navigator Job Duties:

· Provide individual and group therapeutic and experiential interventions.

· Provide Child and Adolescent needs and strengths assessments for drop-in youth.

· Provide supervision of youth in the drop-in center.

· Event coordination and resource tabling for internal and external community events.

· Care coordination with youth involved in complex systems including Juvenile Justice, DHS, and Medicaid.

· Ensure coordination of wrap around services for clients in MHBHC’s care including community supports.

· Coordination including ordering stocking and facilitating the Family Preservation Center’s food pantry.

· Initiate and write referrals to outside community agencies if appropriate including higher levels of care.

· In person and virtual outreach to community agencies, including acute hospitals, detox facilities, community mental health centers, and other community partners for youth.

· Maintains relationships with community partners to ensure referral streams and a strengthened community safety nets.

· Coordinate with client’s RAE, when appropriate, to ensure care.

· Knowledge and ability to navigate and utilize MHBHC’s electronic health record.

· Coordinating with prescription assistance programs

· May be asked to drive company van to transport clients.

· Provide services and manage time in order to meet goals within established timeframes.

· Participate in program, agency, and external meetings and trainings.

· Respond to crisis and emergency calls.

· Act with utmost professionalism, responding promptly and respectfully to all inquiries, suggestions, requests, and complaints in accordance with agency policy.

· Maintain appropriate boundaries with participants, observing all confidentiality and HIPAA protocols.

· Maintain excellent written records and required statistical forms, as well as,

· Administrative paperwork such as time sheets, mileage sheets, expense reports, and disbursement requests.

Adolescent Care Navigator Qualifications:

· Bachelor’s Level education in the field of social work, criminal justice, or counseling and ability to obtain A Certified Drug and Alcohol Counselor (CAT, CAS) is preferred.

· Experience, knowledge of, and sensitivity to the needs of children and youth specifically youth who have experienced abuse and neglect.

· Experience in therapeutic and skills based interventions for youth, specifically experiential work to include play and community based activities.

· Ability to apply positive engagement, motivational, and leadership skills in a culturally appropriate manner, working with adults and their children from diverse and multiply challenged backgrounds.

· Experience in care coordination work and facilitating relationships with professional agencies.

· Verbal and written communication skills, including detail-minded record-keeping and excellent computer skills.

· Be able to work independently as well as part of a team, with good personal boundaries and appropriate use of authority.

· Ability to hear and process critical feedback, as well as, provide it to others.

· Ability to meet deadlines and follow through on commitments.

· Flexibility, creativity, positive attitude, and good humor.

· Valid driver’s license and reliable transportation.

· If offered position, must pass required criminal background check, and have a driver’s license and vehicle.
