Company Overview
More Maitri, Inc. is a non-profit human service agency dedicated to providing supports and services to persons with developmental and intellectual disabilities. Founded on the principle of loving kindness towards all, the organization emphasizes compassionate care and community integration.

Position Summary

The Residential Monitoring and Supports Coordinator is responsible for supporting the quality, documentation, coordination, and day-to-day oversight of residential services within the agency’s Host Home Program and related residential supports. This position conducts in-home monitoring of host homes, completes and reviews monitoring and provider documentation, maintains residential records in both electronic and hard-copy systems, supports service coordination for residents/consumers, attends and helps facilitate service meetings, and provides administrative and program support to the Residential Director. The position supports services that are person-centered, health and safety focused, organized, and consistent with agency policy and applicable HCPF and DD Medicaid waiver standards and regulations.

Essential Duties and Responsibilities

Residential Monitoring and Oversight

Conduct scheduled and assigned in-home monitoring visits of host homes in accordance with agency protocols and assigned monitoring schedules.

Observe and assess the home environment for general health, safety, cleanliness, accessibility, emergency preparedness, and support of resident/consumer needs.

Identify concerns, risks, missing documentation, or follow-up needs and communicate findings to the Residential Director and other designated leadership.

Support follow-up on corrective action items related to monitoring findings, documentation deficiencies, and health and safety concerns.

Assist in verifying that residential settings and service practices are aligned with person-centered expectations, participant rights, and agency standards.

Documentation, Records, and File Management

Complete monitoring forms, visit notes, follow-up documentation, and other required records accurately, professionally, and within required timeframes.

Review host home provider documentation for completeness, timeliness, and compliance with agency expectations.

Scan resident/consumer appointment records and other relevant documentation into the agency’s online platform.

Maintain organized hard-copy residential files and file paperwork in the agency’s on-site filing system.

Assist with retrieval, organization, and preparation of records for internal review, audits, case coordination, and leadership reporting.

Provider, Resident, and Service Coordination Support

Assist host home providers with residents/consumers as needed and within the scope of agency expectations and role responsibilities.

Communicate with host home providers regarding missing, incomplete, or overdue documentation and support follow-up as directed.

Attend resident/consumer service meetings, interdisciplinary meetings, and other residential coordination meetings as assigned.

Help facilitate service meetings by gathering documentation, sharing relevant observations, and supporting follow-up action items.

Assist with obtaining, organizing, and communicating information related to resident/consumer services, supports, appointments, needs, and follow-up care.

Administrative and Program Support

Support the Residential Director in obtaining information and documentation needed for reports, including HRC-related materials and residential service review needs.

Help prepare, compile, track, and organize data or records relating to resident/consumer services, provider follow-up, and residential oversight activities.

Support special projects, corrective action follow-up, audit preparation, and other administrative needs of the Residential Director.

Perform other related duties as assigned in support of residential program quality, continuity, and compliance.

Compliance Responsibilities

Perform duties in accordance with agency policy and procedure.

Maintain confidentiality of resident/consumer, provider, and agency information.

Support services in a manner consistent with HCPF requirements, DD Medicaid waiver standards, and agency expectations.

Recognize and promptly report concerns related to health and safety, abuse, neglect, exploitation, rights restrictions, service gaps, or documentation noncompliance.

Maintain professional documentation and communication standards at all times.

Minimum Qualifications

High school diploma or GED required.

Associate degree or higher in human services, social work, psychology, healthcare administration, special education, or a related field preferred.

Experience working with adults with intellectual and developmental disabilities, behavioral health needs, or Medicaid waiver services preferred.

Experience with residential services, host home services, documentation review, case coordination, or compliance monitoring preferred.

Valid driver’s license, acceptable driving record, and reliable transportation required if travel between host homes or agency locations is expected.

Knowledge, Skills, and Abilities

Ability to conduct in-home monitoring in a professional, respectful, and observant manner.

Ability to complete accurate, timely, and objective documentation.

Ability to organize records in both digital and paper formats.

Ability to communicate clearly and professionally with providers, residents/consumers, team members, and stakeholders.

Ability to identify compliance concerns and elevate issues appropriately.

Ability to manage multiple tasks, deadlines, and follow-up items while maintaining confidentiality.

Work Environment and Physical Requirements

Work is performed in office settings, residential homes, and community-based environments.

Regular travel to host homes and agency locations may be required.

Must be able to sit, stand, walk, bend, carry files, and use office equipment as needed.

Must be able to review residential environments and complete documentation in both field and office settings.

Emergency Response Availability

This position may be required to respond to resident or Host Home Provider emergencies outside of typical business hours, as needed, to support health, safety, program continuity, and compliance with agency expectations and applicable HCPF and DD Medicaid waiver standards. Response may include phone consultation, coordination with the Residential Director or other agency leadership, documentation follow-up, and in-person support when necessary and appropriate.

Pay: From $23.00 per hour
