Overview
We are seeking a dedicated and compassionate Registered Nurse or Licensed Practical Nurse to join our healthcare team. The ideal candidate will be responsible for providing high-quality patient care in a variety of settings. This role requires strong clinical skills, the ability to work collaboratively with other healthcare professionals, and a commitment to patient advocacy.

Duties

Assess and monitor patient health status, including vital signs and symptoms.
Administer medications and treatments as prescribed by physicians.
Perform venipuncture and other necessary procedures for patient care.
Collaborate with interdisciplinary teams to develop and implement individualized care plans.
Provide education and support to patients and their families regarding health conditions and treatment options.
Participate in discharge planning to ensure continuity of care post-hospitalization.
Engage in health coaching to promote wellness and disease prevention among patients.
Provide specialized care in areas such as geriatrics, hospice palliative medicine, and dialysis as needed.

Skills

Proficient in clinical skills such as venipuncture, discharge planning, and patient assessment.
Knowledge of hospice palliative medicine practices is advantageous.
Strong communication skills for effective patient education and collaboration with healthcare teams.
Ability to provide compassionate care to diverse patient populations, including geriatrics.
Familiarity with health coaching techniques to support patient engagement in their health journey.
Capability to work effectively under pressure in fast-paced environments.
