Full-time position that follows the West Des Moines school schedule, Aug 1- June 15. June 16-July 31 off.
The Intensive Care Coordinator provides wraparound interventions for high school students with complex social, emotional, and behavioral needs. Operating under a trauma-informed and strengths-based model, the Intensive Care Coordinator will partner with school teams, families, and community providers to provide individualized care plans and secure vital resources for at-risk youth.

The role of Intensive Care Coordinator is clearly defined by a set of responsibilities which provide identified students with the best possible services available.
● Coordinates, facilitates and monitors the Interdisciplinary Team planning process and assures appropriate implementation of the plan
● Links members to necessary resources and facilitates maintenance of their benefits
● Works as a liaison with collateral systems to resolve conflict, advocate for youth rights and follow-up with specific problem outcomes
● Assists youth and their families in maximizing their abilities for self-direction by supporting them and empowering them to their greatest extent possible
● Encourages and assists youth in maximizing their mental and medical health recovery
● Processes requests for services by obtaining necessary assessments, conducts intakes, completes required assessments and determines member eligibility of services
● Monitors behavioral and medical service provision progress or obstacles
● Provides intensive casework for youth to obtain and retain needed services
● Mediates conflicts related to youth education or services and advocates for the youth when needed
● Completes file audits and other quality assurance activities to ensure work meets requirements of the agency, state and regulatory bodies, school and funding streams
● Functions as a member of a team and as part of the school system to ensure identified youth receive the support they need for safety and stability while at school 2
● Acts as an advocate for the student's rights and educational needs, including collaborating on IEP and 504 plans as needed.
● Conducts home visits as needed to assess barriers and discuss supports needed for youth to attend school consistently.
The Intensive Care Coordinator is directly responsible to and supervised by the Director of Community Based Services or designee. The Intesive Care Coordinator does not have any specific supervisory responsibilities.
Travel is required as needed to and from the service location. The Intensive Care Coordinator will have two main work locations, one at 1516 Valley West Drive, within the Community Based Services Program of CSA, and one within the School they are placed within. Student support visits may occur in the Individual/family home, community setting or office if required. Service location is usually determined based on the need of the youth and family.

Required educational experience for this position includes a Bachelor’s Degree from an accredited college or university with at least 30 semester hours or its equivalent in psychology, sociology, education, human services, criminal justice, education, health care, human service administration, or the social sciences; or have an Iowa license to practice as a registered nurse.
Nurses must have at least three years of experience in the delivery of nursing or human services to the population group; others must have a minimum of one-year experience in the delivery of human services, specific to the needs and abilities of the population served.
This person must possess the ability to: work alone and with limited supervision, manage time effectively to meet deadlines, review multiple standards, interpret information and be detail oriented. This person must qualify with the company’s underwriter as an insurable driver and possess legal driving privileges in the State of Iowa. The individual must clear annual background checks including, but not limited to, criminal, abuse and Office of Inspector General. This person must be certified free of tuberculosis by a medical professional.
The Intensive Care Coordinator should have the ability to communicate effectively to customers and employees of the organization while demonstrating a high level of motivation. Ability to maintain professional boundaries. Demonstrate a high level of detail orientation and organization. Possess good problem-solving skills and be able to follow through with assignments. Ability to read and interpret documentation, analyze and interpret data, use routine computer programs, to develop routine reports, and to handle non-routine crisis situations.
Licensure is not required for this position. Nurses must submit a copy of their current license.

Pay: $23.00 - $25.00 per hour
