Full Job Description

PsyGenics is seeking an outgoing, self-motivated and enthusiastic individual to join our team as a part-time Member Care Specialist! This position is designed to assist our member population in securing needed supports including "shopping" our in-house boutique with their parents and guardians.

Qualifications

The Member Care Specialist must possess a High School Diploma, Associates of Applied Science (A.A.), or a Bachelor’s degree in social or behavioral health and have a minimum of two years relevant experience working with Developmental Disabilities, persons with a Traumatic Brain Injury, or with persons who have Severe Mental Health Issues. Person must have police and DHS clearance. Additional requirements include: a nonjudgmental approach that reflects cultural competency and sensitivity to the unique needs and diverse experiences of individual families, the ability to engage and maintain clinical relationships with families, the ability to develop collaborative relationships with other providers, effective interpersonal communication skills, the ability to assess and intervene within complex family and community systems, and a willingness to be flexible, patient and creative.

Position Summary

The Member Care Specialist operates as a part of the Case Management Team to ensure that all needs of a member are provided by linking, coordinating, advocating, negotiating and referral services under the guidance of a Case Manager and Clinical Program Manager. The Member Care Specialist is responsible for ensuring the implementation of the Person-Centered Plan to the specifications of the member as well as the monitoring of the established goals and the implementation of various services in conjunction with the multi-disciplinary team. The Member Care Specialist will act as an advocate for the member, monitor the member’s experience, continually evaluate the member’s engagement and problem solve any barriers to receiving services as medically appropriate and in conjunction with PsyGenics’ Mission to improve the health and wellness of the community served.

Specific Responsibilities

Assesses member’s basic needs and links to in-house and community resources including but not limited to personal care items, clothing, household goods, etc. through PsyGenics Heart to Hanger Boutique and other resources.

Creates attention grabbing posters and flyers using AI and other software tools
Ensures boutique is neatly organized
Rotates seasonal items in/out of boutique
Delivers stock that is not moving to another donation center
Assists members with shopping
Teaches members as appropriate marketing and merchandising skills
Maintains inventory of boutique items
Creates and posts flyers promotes use of boutique
Ensures delivery and monitoring of services as prescribed in the Person-Centered Plan
Reviews critical documents with members to ensure understanding and secures required signatures
Completes member experience surveys with members to provide feedback on service and service delivery improvement to the clinic on behalf of the member and community
Reviews the individualized plan of service based on the comprehensive assessment which includes goals established by all appropriate disciplines to ensure member understanding and approval
Assists Case Manager in the delivery of referrals, services and implementation of the Person-Centered Plan
Coordinates and completes all necessary referrals on behalf of the member
Develops and maintains written records necessary for professional practice as defined by policy and procedure
Provides outreach services to members and their families in their respective locales and/or homes
Attends DWIHN meetings and trainings as required
Provides community education services as assigned
Participates in clinical supervision, team meetings and case consultations as appropriate
Must be knowledgeable of the Michigan Mental Health Code and Medicaid guidelines as well as billing protocols and procedures
Obtains all prior authorizations and applicable coordination of care
Follows PsyGenics, Inc. professional code of conduct
Represents PsyGenics, Inc. in the community
Performs similar or related duties as assigned

This is a part-time position - Monday - Friday from 9:0AM - 2:00PM. The position is not remote.
