Full Job Description

GENERAL SUMMARY

7PM TO 7:30AM

The Registered Nurse (RN) provides high quality nursing care to patients using the nursing process. The registered nurse assesses patients with mental health concerns and implements treatment plans while collaborating with the interdisciplinary treatment team to assist the patient in recovery and wellbeing in a safe and therapeutic environment.

PRINCIPAL DUTIES AND RESPONSIBILITIES

1. Nursing Practice and Patient Care Activities:

A. physical, psychological and social dimensions of patients.

1. Applies knowledge of illnesses, injuries, and diseases and uses available resources in assessment process.

2. Conducts comprehensive nursing assessments to evaluate the patient’s mental, emotional, and physical state.

3. Monitors vital signs and psychiatric and medical symptoms documenting any changes and responding with appropriate treatment.

4. Provides written assessment of patient on admission and daily thereafter.

5. Summarizes assessment data and sets goals and expected outcomes which are documented and communicated to other patient care providers.

1. Initiates plan based on assessment data and applies knowledge of specific illnesses, injuries, diseases, human behavior and appropriate standards of care.

2. Develops, implements, and updates individualized care plans in collaboration with the patient and the treatment team.

3. Sets achievable goals with the patients and modifies care plans as needed based on the progression of the patient

4. Documents specific nursing interventions and responses to care and communicates with their psychiatrist and the treatment team.

5. Utilizes all available resources in planning care and consults as necessary with members of the treatment team.

6. Updates plan of care as necessary based on continuing assessments of patients.

7. Participates in discharge planning in conjunction with other disciplines.

1. Implements clinical and technical aspects of care and nursing orders in accordance with established policies and procedures. Intervenes with proper technique, procedure and safety precautions to meet individual needs of patient.

2. Delegates duties to appropriate nursing staff.

4. Utilizes a available resources

6. Respects dignity and confidentiality of patient, serving as an advocate as necessary and providing a safe and secure environment for the patient.

7. Functions within limits of state regulations, hospital nursing department policies and procedures and Standards of Practice.

II. Patient and Family Instruction/Education and Discharge Planning

2. Identifies needs for care at home, or alternatives such as outpatient treatment, extended care facility, etc., in conjunction with other disciplines.

2. Performs within limits of State regulations, Nurse Practice Act and hospital policies and procedures.

C. Acts as a resource person and role model for patients and staff.

D. Functions as the Charge Registered Nurse when assigned, providing leadership and direction to staff respectfully and assuring the provision of care to all patients on the unit.

E. Promotes positive working relationships amongst staff and between other disciplines and departments.

F. Participates in nursing and hospital committees and other activities which promote the growth and development of nursing and/or patient care.

G. Participates in quality assurance and performance improvement initiatives to enhance patient care and safety.

H. Remains informed about best practices and current trends in psychiatric nursing and mental health care.

I. Creates daily assignment sheet for staff at the start of the shift so staff may know assigned patients.

J. Leads Safety Huddle twice during the shift and ensures all available staff attend for updates.

K. May be assigned as the Charge Registered Nurse for the shift.

L. Responds to crisis situations effectively to ensure staff and patient safety,

M. Provides direction to staff in managing challenging patient behaviors in compliance with established procedures.

N. Maintains appropriate documentation of events which have occurred during the shift, such as occurrence and incident reporting.

O. Ensures completion of nursing documentation for restraint, seclusion, restrictive interventions, care plans, and prn medications have been completed.

SUPERVISION:

Reports to the Nurse Manager. The registered nurse may supervise other registered nurses, licensed practical nurses, Nursing Assistants, Mental Health Technicians, Undergraduate Nurse Externs, and other nursing staff.

KNOWLEDGE, SKILLS AND ABILITIES:

1. Associates Degree in Nursing required. A Bachelor's degree in Nursing preferred.

2. Minimum 1 year in a behavioral health position.

3. : Ability to effectively communicate with patients, families and hospital personnel. Expected to float or be pulled to any other area as needed.

4. Clinical knowledge and analytical abilities necessary to formulate effective nursing care plans. Ability to perform essential professional nursing services as defined for their respective area. Completion of orientation and educational program as established for particular work area.

5. Registered Nurse License in the State of Michigan, BLS certification and CPI certification.

WORKING CONDITIONS:

1. Long periods of standing, walking, bending, lifting, pushing and pulling.

2. Works in a well-lighted, well-ventilated environment.

3. Medical/office equipment necessary to perform specific tasks/duties.
