About the Role
This hybrid position consists of approximately 50% Case Management and 50% Designated / Program Coordination responsibilities.
As a Hybrid Case Manager / Designated Program Coordinator, you will coordinate services for individuals and families while also providing leadership and oversight for residential and community support programs. You'll work collaboratively with individuals receiving services, families, Direct Support Professionals (DSPs), leadership, and community providers to ensure quality, person-centered supports are delivered in compliance with state and organizational standards.
This role is ideal for someone who enjoys building relationships, solving problems, mentoring staff, coordinating services, and helping individuals achieve meaningful goals.
What You'll Do
Case Management Responsibilities:
- Coordinate services and supports for individuals and families.
- Develop person-centered support plans based on assessed needs, goals, and preferences.
- Monitor and evaluate services to ensure individual needs continue to be met.
- Assist individuals in accessing community resources, programs, and services.
- Help identify and connect individuals with qualified service providers.
- Educate individuals and legal representatives about available service options.
- Advocate for self-direction, independence, and community inclusion.
- Maintain accurate case documentation and ensure timely completion of required records.
Designated / Program Coordinator Responsibilities:
- Coordinate and participate in support team meetings to align services, goals, and individual needs.
- Collaborate with Directors, support teams, families, and outside providers to ensure high-quality person-centered services.
- Design, implement, and monitor individualized support programs.
- Train, mentor, coach, and support House Leads and Direct Support Professionals (DSPs).
- Ensure compliance with Service and Support Plans, company policies, Minnesota DHS regulations, and 245D standards.
- Monitor program quality and identify opportunities for continuous improvement.
- Maintain accurate documentation and program records.
- Respond effectively to changing needs, operational priorities, and crisis situations when necessary.

Qualifications
Required:
- Bachelor's degree from an accredited college or university in Social Work, Psychology, Sociology, Human Services, or a closely related field.
- Valid driver's license.
- Reliable, insured transportation.
- Ability to successfully pass a Minnesota DHS Background Study (245D).
Preferred:
- One year of direct care or human services experience.
- Experience coordinating services or supervising staff.
- Knowledge of Minnesota DHS regulations and person-centered planning.
