About the Role
As a Designated / Program Coordinator, you will play a key leadership role in coordinating and overseeing support services that promote independence, dignity, health, and personal growth for individuals receiving our services.
You’ll work closely with leadership, Direct Support Professionals (DSPs), families, and support teams to ensure person-centered services are delivered effectively, safely, and in compliance with state and organizational standards.
This role is ideal for someone who is organized, compassionate, solution-focused, and passionate about leading teams while improving quality of care.
What You’ll Do
In this role, you will:
- Coordinate and participate in support team meetings to align services, goals, and individual needs
- Collaborate with Directors, support teams, and families to ensure high-quality person-centered planning
- Design, implement, and monitor individualized support programs
- Train, mentor, and support House Leads and Direct Support Professionals (DSPs)
- Ensure compliance with Service and Support Plans, company policies, and regulatory standards
- Provide coaching, guidance, and ongoing support to team members
- Maintain accurate, timely, and organized documentation
- Identify opportunities for improved service delivery and positive client outcomes
- Respond effectively to changing needs and operational priorities

Essential Qualifications
- Must successfully pass a Minnesota DHS Background Study (MN 245D)
- Valid driver’s license
- Reliable, insured transportation
- Strong communication, organizational, and computer skills
- Ability to effectively lead, mentor, and support Direct Support Professionals
Education & Experience Requirements
Applicants MUST meet one of the following qualifications:
- Bachelor’s degree in a human services-related field and 1 year of direct care experience
- Associate degree in a human services-related field and 2 years of direct care experience
- Diploma in a human services-related field and 3 years of direct care experience
OR
- Minimum of 50 hours of human services/disabilities-related training AND 4 years of full-time direct care experience under appropriate supervision
