Tidewell Health Management Systems is seeking a Texas-licensed Psychiatric Mental Health Nurse Practitioner (PMHNP) interested in becoming a founding provider in the development of a new outpatient behavioral health practice.
This is a startup-stage opportunity with a planned launch before the end of 2026. No capital investment is required from the provider. Tidewell will develop and support the business and operational infrastructure necessary to launch and grow the practice.
We are not recruiting someone to inherit an established full patient panel. Instead, we are looking for a clinician who is excited about joining early, beginning with a limited and flexible schedule, and gradually expanding their availability as their patient panel and the practice grow.
This may be particularly attractive to a PMHNP who has considered private practice but does not want to independently take on the financial investment, administrative burden, and operational complexity of building one from scratch.
How We Plan to Start
The practice will launch intentionally lean.
Initially, providers will be able to open a limited number of appointment hours and gradually build their schedules as patient demand develops. There is no expectation that a provider immediately leave an existing position or begin with a full-time caseload.
The initial model is expected to have a significant telepsychiatry component. Our first physical location is planned for the Clear Lake / League City area, with permanent clinical space established as patient volume and future service lines justify the investment.
Our goal is to build the practice responsibly rather than incur unnecessary overhead before the patient base exists to support it.
What Tidewell Provides
Tidewell Health Management Systems will focus on developing and managing the non-clinical infrastructure necessary to build and scale the practice, including:
- Practice administration and operations
- Credentialing and payer enrollment coordination
- Billing and revenue-cycle infrastructure
- EHR and practice technology
- Scheduling and patient-access systems
- Marketing and patient acquisition
- Referral-development strategy
- Operational and financial analytics
- Administrative staffing as volume develops
- Development of the Clear Lake / League City clinical location
- Operational development of additional behavioral health service lines
The objective is simple: allow clinicians to focus primarily on patient care while Tidewell focuses on building and operating the business infrastructure around them.
Long-Term Vision: TMS & Spravato
The practice will initially focus on outpatient psychiatric evaluation, medication management, and telepsychiatry.
As the patient population and infrastructure develop, Tidewell intends to expand the practice into Transcranial Magnetic Stimulation (TMS) and Spravato (esketamine) treatment services.
These programs would provide additional treatment options for patients with treatment-resistant depression while creating additional service lines and revenue opportunities for the growing practice.
Implementation will occur as patient volume, clinical leadership, payer participation, regulatory requirements, and physical infrastructure allow.
Founding Provider & Strategic Partnership Opportunity
This is intended to be more than simply filling shifts in an established clinic.
Tidewell is looking for clinicians who are interested in helping build a practice from its earliest stage and potentially developing a long-term strategic relationship with the organization.
Tidewell will focus on the management and operational side of the business, while the clinical practice will be established through an appropriately structured clinician-led entity with independent clinical governance and clinical decision-making authority.
For the right founding provider, there may be opportunities to participate more meaningfully in the development, leadership, and long-term strategic direction of the clinical organization as the practice grows.
The specific clinical and business structure will be developed in accordance with applicable Texas legal, regulatory, and professional requirements.
No capital investment is required to participate as a founding provider.
Who We're Looking For
The ideal candidate:
- Is a Texas-licensed, board-certified PMHNP
- Has experience with outpatient psychiatric medication management
- Values clinical autonomy and flexible scheduling
- Is comfortable beginning with a limited schedule and building gradually
- Is interested in developing a long-term patient panel
- Has an entrepreneurial mindset
- Is interested in helping shape a new practice rather than simply joining an established one
- Has strong communication and documentation skills
- Practices evidence-based, patient-centered psychiatric care
- Has an interest in treatment-resistant depression and innovative psychiatric services
Experience with TMS and/or Spravato is welcomed but not required.
An established patient panel is also not required.
Schedule & Compensation
This opportunity is expected to begin as a 1099, production-based arrangement with substantial scheduling flexibility.
Providers can initially maintain a limited schedule while continuing other employment and expand their availability as their patient panel develops.
Compensation will correspond with actual patient volume and productivity. At a mature caseload, estimated annual compensation may be approximately $140,000–$200,000+, depending on schedule, productivity, payer mix, reimbursement, and patient volume.
This range represents potential compensation with a developed caseload and is not a guaranteed starting salary.
