Case Manager- Supportive Housing
Tuesday-Saturday 12:00pm-8:00pm.
POSITION SUMMARY/FUNCTION
Responsible for providing care coordination / case management services to those with mental illness, substance use and/or developmental disabilities. Conduct interviews and assist in accessing appropriate community services. Maintain appropriate patient records and tracking of patients between agencies. Some travel may be required within the catchment area.
ESSENTIAL DUTIES/RESPONSIBLITIES/FUNCTIONS/TASKS:

Provide initial care coordination/case management services and develop written service plans for each patient.

Conduct face to face interviews (where appropriate) with patients and/or patient representative to assess individual needs, supports, goals, etc.

Information gathered may include but not limited to: identifying information, community living, employment, physical needs, health, social history, housing and physical environment, socialization and recreation.

Identify appropriate community service needs of patients and determine appropriate community resources to meet those needs.

Involve the patient and/or family in the development of the written service plan and conduct in a timely manner.

Effectively implement service plans for patients.

Coordinate service delivery to patients among appropriate agencies as specified in the service plan.

Provide family support and/or basic living skills as needed.

Conduct periodic assessment and modification of service plan.
-
Advocate on behalf of patient and target population as a whole regarding social, economic, and residential requirements.

Conduct home visits.

Provide transportation only when it is a necessity. Ensure all community resources such as family, friends, public transportation and special vehicles are utilized to meet the needs of patient. If transporting clients in personal vehicle, must maintain additional liability insurance as required.

Coordinate care for patients between agencies and monitor/follow up with the patient and agency regarding this care.

Close cases as appropriate.

Meet required productivity level of 80% direct service time.

Assist with maintaining patient records in a professional manner and in accordance with all SpectraCare policies and procedures, Alabama Administrative Code, CMS, and all other applicable regulating and contractual entities.

Ensure privacy and confidentiality of all patient records in accordance with HIPAA, 42 CFR and all other applicable state or federal laws.

Ensure all written documentation is completed accurately and in a timely manner.

Ensure that all records are staffed within standard time frames, with service plan revised as needed and in accordance with regulatory requirements.

Ensure that all progress notes are completed promptly, written in a professional manner and in accordance with policies and procedures.

Ensure that goals are appropriate, specific, and the focus of treatment.

Ensure that all patient information is kept up to date.

Ensure patient release of information forms are signed in a timely manner and maintained within the chart.

Administrative and other related duties as assigned to include but not limited to:

Serve as a liaison and advocate for SpectraCare patients with community agencies and the public.
-
Assist with ensuring the care coordination/case management services remain within budget parameters.
-

Completes assigned tasks in a timely manner.

Works in a cooperative manner with other SpectraCare employees.

Receive and respond to inquiries promptly and courteously.

Maintain travel records accurately and submitted within the appropriate timeframe.

Ensure necessary documentation is completed and up to date before accrued leave is taken.

Follow guidelines of SpectraCare Personnel Policies, SpectraCare Clinical Policies, Alabama Department of Mental Health Administrative Code, CMS, and all other applicable regulating and contractual entities.

Fulfill duties of Nurse Delegation Program.
-
Become fully certified as a Medication Administration Certified (MAC) assistant.
-
Follow all regulations as outlined in the MAC policies including but not limited to the Seven (7) Rights and Responsibilities, training, consultation with RN/LPN, error reporting, etc.
-
Comply with all documentation requirements of the MAC program including the MARS, counting and recording controlled substances, PRN medications, sign out sheets for home visits, and medication errors reporting sheets.

Regular attendance with timely arrival and departure according to scheduled hours.
-
Must be able to work full time with rotating assignments as necessary to include but not limited to overtime hours, nights, and/or weekends as scheduled.

To ensure client/employee safety while on duty, must remain awake during scheduled working hours.

Comply with SpectraCare’s attendance policy.

Attend mandatory trainings as required by ADMH or other agencies.

ESSENTIAL QUALIFICATIONS/KNOWLEDGE/SKILLS/ABILITIES:

Knowledge of developmental, substance use and/or mental illness disorders.

Ability to design, implement/administer individualized behavioral and/or developmental services in regard to case management services.

Ability to provide services in all facilities and other locations as deemed necessary.

ESSENTIAL TRAINING REQUIREMENTS:
-
Complete initial new hire training.
-
Complete required on the job training (OJT) training by the department.
-
Obtain proper certifications needed to perform job duties.
-
Complete annual training as required by the Alabama Administrative Code, CMS, and all other applicable regulating and contractual entities.
-
Complete Care Coordination / Case Management Training as required by the Alabama Department of Mental Health
-
If working in the Developmental Disability Division, for purposes of facilitating Person-Centered Planning meetings and service coordination, QDDP must have at least one year of experience working directly with persons with developmental disabilities, have successfully completed a four-day Personal Outcome Measures (POM) workshop and co-facilitated a Person-Centered Planning meeting with ADMH staff.

Obtain TB skin test and/or TB Assessment prior to initiation of duties after hiring and annually thereafter.
-
Complete First Aid/CPR upon hire and working alone with clients; and continuously thereafter maintaining current certification.
WORK ENVIRONNMENT / PHYSICAL REQUIREMENTS:
-
Physical ability to work assigned shift. May require periodic weekend/evening work to meet the organizations’ needs.
-
Must be able to travel to system facilities and/or other locations/outings with exposure to changing weather conditions.
-
Work location is primarily in a temperature-controlled setting with minimal periods of sitting at a desk with associated repetitive motions. Must understand and practice good ergonomic movements to avoid injuries.
-
Physical ability to correctly lift and/or moves up to fifty (50) pounds on an infrequent basis.
-
Required to talk to or hear staff, individuals served, and/or others.
-
Required to stand, walk, sit, bend, twist, kneel, and/or reach frequently.
-
Physical ability to perform CPR.

Employee may be exposed to one or more of the following: disagreeable odors, chemicals such as disinfectants, cleansers, soaps, etc; latex, plastic and other materials used for personal protective equipment; noise and distractions; unpleasant sights; unpredictable behaviors; contagious diseases; and/or wet floors.

Required to train professional staff through verbal and visual presentations.

REQUIRED QUALIFICATIONS/EDUCATION/EXPERIENCE:

Bachelor’s degree in psychology, social work, human services and/or related field for all CM positions (MHSU/DD).

Must have a valid driver’s license.

Good MVR acceptable to automobile insurance carrier for inclusion on agency policy.
-
Must maintain automobile liability insurance.

If working with the Developmental Disability population, one-year experience working directly with persons with developmental disabilities and complete QDDP training and POM training to facilitate Person-Centered Planning meetings. This individual must be a registered nurse, hold bachelor’s degree in human services field, or a bachelor’s degree with 12 hours successfully completed hours from an accredited institution in human service’s field.
PREFERRED QUALIFICATIONS/EDUCATION/EXPERIENCE:

Prior case management experience in a mental health/developmental disability setting.
