POSITION DESCRIPTION
TITLE: Community Health Worker Liaison: Haitian-Creole
DIVISION & PROGRAM: Women’s Health Screening
REPORTS TO: Chief Operations Officer
EMPLOYMENT STATUS: Full-time and Exempt
SUMMARY:
The Community Health Worker Liaison is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to social determinant of health services and improve the quality and cultural competence of service delivery. The Community Health Worker Liaison also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy.
As a member of the Women’s Health Screening Team, the Community Health Worker Liaison will be responsible for identifying social, emotional, environmental, financial, and other support needs for community members and established constituents while connecting them with appropriate services and community resources. This position will work closely with our Women’s Health Screening and Survivorship teams and community partners to identify areas of need and resources with the goal of providing breast screenings and improving patients’ health care and outcomes.
MINIMUM QUALIFICATIONS:

- BA degree preferred or equivalent work experience in education, health-related field, social sciences, or social work.
- Bilingual – Haitian-Creole language oral and written communication required.
- Community Health Worker Certification (preferred).
- Proven community development skills and knowledge of working with diverse communities.
- Knowledge of working with diverse communities.
- Demonstrated competence in Microsoft Office programs.
- Excellent interpersonal, writing, collaboration, and problem-solving skills. Must be a team-player.
- Valid driver’s license and excellent driving record required.
KNOWLEDGE OF:

ESSENTIAL POSITION REQUIREMENTS:
- Traveling between offices and communities in Delaware to conduct meetings and outreach events with constituents in need of breast health resources to provide case management by completing needs assessment, identifying social needs, developing effective service plans, collaborating with healthcare systems, coordinating care and connecting to appropriate internal and/or external community resources.
- Assist individuals and families in the community in accessing health and psycho-social related services, including but not limited to obtaining a medical home, providing instructions on appropriate use of the medical home, overcoming barriers to obtaining needed medical care i.e. clinical breast exams, behavioral health and social services while reducing cultural and socio-economic barriers between clients and institutions
- Facilitate community outreach and public education by participating in community events and conducting presentations in the community as needed.
- Maintain accurate and timely documentation.
- Cultural mediation among individuals, communities, and health and social service systems (e.g., educating individuals and communities about how to use health and social service systems, building health literacy, and cross-cultural communication).
- Provides culturally appropriate health education and information (e.g., conducts health promotion and disease prevention education in a manner that matches linguistic and cultural needs of participants and/or communities).
- Care coordination, case management, and system navigation (e.g., makes referrals and provides follow-up, facilitates transportation services and helps to address other barriers to services; documents and tracks individual and population level data).
- Assists in creating a positive and supportive work environment; enforces a safe workplace; establishes a culture of teamwork and communication; creates a workplace that promotes the organizational values of workplace diversity, equity and inclusion and actively promotes an environment respectful of living and working in a multicultural society.
- Provides coaching and social support (e.g., motivates and encourages people to obtain care and other services, supports self-management or disease prevention and management of health conditions).
- Advocates for individuals and communities (e.g., advocates for the needs and perspectives of communities, connects to resources, and advocates for basic needs).
- Interviews clients to obtain basic data, past medical history, and determines income status/program eligibility.
- Develops and maintains working relationships with clients and makes referrals to other community agencies and services.
- Participates in evaluation and research (e.g., engages in evaluating CHW services and programs).
- Performs other related duties as requested or required.
WORKING HOURS:
The agency has general operating hours Monday – Friday, between 9:00am – 5:00pm with flexibility, although necessary work hours will be scheduled on evenings and weekends depending on the needs of the program and agency. This position is a hybrid of office and work at home hours.
