Full Job Description

JOB TITLE: Social Worker

SUMMARY: The Social Worker will be responsible to manage the day to day aspects of the Social Services.

ESSENTIAL DUTIES AND RESPONSIBILITIES: This summarizes the major essential duties and responsibilities regularly performed and normally assigned to this position. Listed below are the duties in descending order of importance and are as clear and specific as possible. Other duties may be assigned. May be necessary to assist in the evacuation of residents during emergency situations.

Manage a smooth discharge plan for all residents that begins on day of admission with other departments
Manage care conferences and coordinate department schedule and family attendance
Complete MDS / Care Plans for Social Service sections
Coordinate all insurance last covered day dates, provide appeal rights to residents/family members and assist with the insurance appeal process, if needed
Complete clinical Social Service assessment on all admissions timely
Coordinate and maintain all code status records for residents in appropriate locations
Assist resident and family members to ensure that all customer service needs are met
Ensure that Durable Medical Equipment (DME) needs are taken care of prior to d/c
Ensure that Home Health Services are arranged for all d/c’s

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and/or abilities required. EDUCATION and/or EXPERIENCE: Bachelor's degree (B. A.) from four-year college or university, preferable degree in Social Work or Human Services with related internship. Minimum of or one to two years’ related experience and/or training.

LANGUAGE SKILLS: Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures or governmental regulations. Ability to write reports, business correspondence and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers and the general public.

MATHEMATICAL SKILLS: Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference and volume. Ability to apply concepts of basic algebra and geometry.

REASONING ABILITY: Ability to define problems, collect data, establish facts and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete, variables.

COMPUTER SKILLS: Proficiency with Microsoft software (Word, Excel, etc.) Familiar with telephone and various office equipment.

OTHER SKILLS AND ABILITIES: Excellent communication and relationship building skills. Ability to effectively manage multiple priorities. Ability to work calmly in a fast paced and quick changing environment.

While performing the duties of this job, the employee will work in a clean, well-lighted and well-ventilated office setting. The noise level in the work environment ranges from quite (private office) to moderate (business office with computers ad office equipment, light traffic).

OSHA EXPOSURE RISK CATEGORY: Level II. Work environment involves limited physical risks or hazardous conditions.

ADDITIONAL INFORMATION: Must possess and exhibit a strong positive attitude toward customer service and have a genuine interest in geriatric care and uphold the philosophy of the community.
