Full Job Description

Lead Clinical Excellence. Shape Quality. Make an Impact.

We are looking for an experienced and driven Director of Patient Services (DPS) to join our leadership team and oversee the clinical operations of our home care agency.

This is a key leadership position responsible for ensuring high-quality patient care, regulatory compliance, staff development, and strong operational performance across the agency. The ideal candidate is a confident clinical leader who understands the home care environment, is knowledgeable in NYS Department of Health regulations, and enjoys building strong teams and improving processes.

This is a full-time, in-person position based in Monsey, NY.

Home care experience is required. LHCSA experience is preferred.

What You'll Do

Lead Clinical Operations: Oversee clinical services, nursing coordination, field nursing, Clinical Supervisors, ADPS staff, and other clinical functions to ensure high-quality patient care.
Ensure Regulatory Compliance: Maintain agency compliance with federal, state, local, and NYS Department of Health requirements, policies, procedures, and standards.
Drive Quality & Performance Improvement: Oversee clinical audits, documentation reviews, quality assurance activities, incident and complaint investigations, patient satisfaction measures, and performance improvement initiatives.
Provide Clinical Leadership: Guide and support clinical staff while identifying opportunities to strengthen nursing coordination, field operations, and internal processes.
Oversee Clinical Documentation: Ensure nursing orders, 485s, and other required documentation are completed, reviewed, corrected, and maintained accurately and timely.
Develop & Mentor Staff: Train and educate agency staff on policies, procedures, forms, and DOH regulations; oversee performance reviews and support ongoing professional development.
Oversee Training & Orientation: Ensure quality execution of PCA training school, staff orientation, and clinical training programs while identifying and addressing gaps in content.
Collaborate Across Departments: Partner with Quality Assurance, leadership, vendors, and other departments to resolve concerns, improve processes, and support agency-wide goals.
Support Continuous Improvement: Participate in leadership meetings, Quality Improvement Committee activities, professional organizations, seminars, and other initiatives that strengthen agency services.
Monitor Quality & Outcomes: Track clinical and quality metrics and prepare reports and recommendations for leadership and the Quality Improvement Committee.
Support Agency Leadership: Take on additional responsibilities and projects as needed to support the agency's clinical and operational goals.

What We're Looking For

Current, unrestricted New York State Registered Professional Nurse (RN) license required
Home care experience required
LHCSA experience preferred
Clinical leadership and supervisory experience
Strong knowledge of NYS Department of Health regulations and LHCSA requirements
Experience with quality assurance, quality improvement, clinical compliance, and patient care oversight
Strong leadership, communication, organizational, and problem-solving skills
Ability to lead, mentor, and develop clinical teams
Ability to collaborate effectively across multiple departments and manage competing priorities
A proactive leader who is committed to delivering excellent patient care and improving clinical operations
