Rapid Resolution Case Manager – SSVF

Location: 896 Main Street, Darby, PA 19023
Schedule: Monday–Friday, 8:00 a.m.–4:30 p.m. or 8:30 a.m.–5:00 p.m.
Salary Range: $48,000–$56,000 annually
Work Arrangement: 100% in-office
Travel: Regular local travel throughout Delaware County and the Philadelphia area

Position Summary

The Rapid Resolution Case Manager provides housing-crisis intervention and supportive services to Veterans and their families who are experiencing homelessness or are at imminent risk of homelessness.

This position conducts coordinated assessments, determines program eligibility, develops housing-stability plans, and connects eligible Veteran households with financial assistance, case management, community resources, and other services designed to prevent or quickly resolve homelessness.

Essential Duties and Responsibilities

Conduct initial interviews and coordinated assessments with Veterans experiencing a housing crisis.
Use problem-solving, landlord mediation, family reunification, and other diversion strategies to help prevent literal homelessness.
Determine eligibility for Supportive Services for Veteran Families services, including verifying income and obtaining required documentation.
Complete intake, assessment, and immediate critical-needs interventions.
Develop individualized housing-stability plans with clear goals and action steps.
Assist Veterans in rapidly exiting homelessness through temporary financial assistance, case management, and referrals to community resources.
Provide trauma-informed, strengths-based, and client-centered services.
Identify barriers related to housing, income, employment, transportation, healthcare, and other stability needs.
Connect Veterans with Continuum of Care programs, public benefits, employment resources, healthcare providers, and other community services.
Maintain accurate and timely case documentation in HMIS and other required program records.
Prepare files and case information for weekly eligibility review and disposition.
Provide brief or “light-touch” assistance to Veterans who do not meet full program eligibility requirements but need short-term support or referrals.
Develop and maintain relationships with landlords, property managers, social-service agencies, and community partners.
Conduct outreach throughout Delaware County and the Philadelphia area to identify and engage Veterans in need of assistance.
Provide follow-up case management to support housing stability.
Complete required program reports and documentation.
Participate in staff meetings, trainings, and case conferences.
Provide coverage and support to other team members as needed.
Perform other duties as assigned.

Position Objectives

Prevent Veterans and their families from entering homelessness.
Help Veterans experiencing homelessness obtain safe and stable housing as quickly as possible.
Provide responsive, respectful, and high-quality supportive services.
Build strong working relationships with landlords and community-service providers.
Empower Veterans through strengths-based and solution-focused case management.

Qualifications

Bachelor’s degree in social work, human services, psychology, sociology, counseling, or a related field required.
Master’s degree in a human-services field preferred.
Relevant experience in housing services, homelessness prevention, case management, or Veteran services preferred.
Knowledge of Housing First, Progressive Engagement, trauma-informed care, and strengths-based practices preferred.
Familiarity with Delaware County and Philadelphia-area social-service systems and community resources strongly preferred.
Excellent organizational and time-management skills.
Ability to manage multiple cases, deadlines, and competing priorities.
Strong written, verbal, and interpersonal communication skills.
Ability to work effectively with individuals from diverse cultural, economic, and social backgrounds.
Proficiency with Microsoft Word and Excel.
Experience with HMIS or a similar case-management database preferred.
Ability and willingness to travel throughout Delaware County and the Philadelphia area.
Valid driver’s license and reliable transportation may be required based on Agency travel expectations.

Physical Requirements

The employee must be able to perform the essential functions of the position, with or without reasonable accommodation. The role regularly requires sitting, standing, walking, speaking, hearing, and navigating stairs. The employee may occasionally be required to lift or move up to 15 pounds.

This position requires regular travel to client residences, community locations, partner agencies, and other worksites throughout Delaware County and the Philadelphia area.
