As an RN Case Manager, you'll build meaningful relationships while leading and coordinating individualized care plans that truly make an impact.

What You'll Do

As an RN Case Manager, you will:

Complete hospice admissions and comprehensive nursing assessments in patients' homes.
Develop, implement, and update individualized plans of care based on each patient's physical, emotional, psychosocial, and environmental needs.
Coordinate care with the interdisciplinary team, physicians, patients, families, and referral sources to ensure exceptional patient outcomes.
Deliver skilled nursing care while following infection prevention and OSHA guidelines.
Educate patients and caregivers on disease progression, symptom management, medications, and available resources.
Maintain timely, accurate documentation in the electronic medical record.
Participate in interdisciplinary team meetings, quality improvement (QAPI) initiatives, and continuing education opportunities.
Support our hospice mission by providing compassionate, patient-focused care at every visit.
Meet productivity expectations while maintaining high-quality clinical standards.
Participate in an on-call rotation to provide support for patients and families when needed.

What You'll Bring

Education & Experience

Graduate of an accredited Diploma, Associate, or Bachelor's School of Nursing.
Current Registered Nurse (RN) license in the state of practice.
Current driver's license with reliable transportation and valid auto liability insurance.
Minimum of two (2) years of clinical nursing experience.
Minimum of one (1) year of full-time hospice experience.

Work Environment

This position is primarily field-based and requires travel to patient homes throughout the service area. You may encounter a variety of home environments and weather conditions and should be comfortable working independently while remaining connected to your care team.

The role may involve:

Local travel throughout the assigned territory.
Exposure to blood, bodily fluids, and infectious diseases while following appropriate safety precautions.
Flexible scheduling, including participation in an on-call rotation.
Occasional evening, weekend, or emergency patient visits.

Physical Requirements

This position requires the ability to:

Stand, walk, bend, and move throughout the workday.
Lift, transfer, or assist patients weighing up to 50 pounds.
Operate a tablet and electronic health record efficiently.
Communicate clearly with patients, families, physicians, and colleagues.
Exercise sound clinical judgment and make independent decisions in a variety of patient care situations.
Travel independently to patient homes, office locations, and educational meetings as needed.
