Social Services Director

Position Overview:

Assists in providing supervision of the Social Services Department to ensure the social, emotional and medical needs of the residents are met in accordance with all laws, regulations and facility guidelines.

Basic Qualifications:

Education and Certification:

MSW or BSW or Human Services degree.

Skills & Requirements:

Must possess the ability to read, write and speak the English language; must have strong communication and presentation skills in both group and individual situations; must be patient, friendly, courteous and respectful to residents in any situation, including difficult ones; must be able to make independent decisions when necessary; must have the ability to follow social services department guidelines as outlined by the facility and other regulations governing social services in a long-term care facility; must be able to move frequently throughout the workday; must be able to lift 20 lbs floor to waist; lift 10 lbs waist to shoulder; lift and carry 20 lbs; and push/pull 20 lbs.

Experience:

Minimum one year experience in a social services position within a health care setting; active license as a Social Worker in current state.

Working Conditions:

Primarily works in an office setting as well as throughout the facility; bends, lifts, sits, stands and moves frequently throughout the shift; deals with frequent interruptions and possibly upset residents and family members; works regularly scheduled shifts with possible weekends, holidays and other hours based on resident needs; primary responsibility is service to the residents but also deals with visitors, government personnel, etc.; participates in required continuing education programs; subject to odors, falls, burns and exposure to infectious diseases, waste, etc., including AIDS and Hepatitis B viruses.

Resident Rights: Promptly reports resident grievances to supervisors; treats all resident care information with confidentiality; continuously monitors assigned areas to ensure residents are treated fairly and with dignity and respect at all times.

Organizational and Communication Skills: Ability to multi-task and is detail-oriented; possesses the ability to effectively prioritize matters promptly and appropriately; understands the importance of follow-up; builds rapport with residents and families through listening attentively and effectively following up; able to effectively communicate individually or as a group; observant and responsive to residents and families; displays an attitude of compassion and sincerity in dealing with resident matters.

Safety Awareness:

Follows safety program guidelines; immediately reports accidents and incidents to supervisors; identifies unsafe working areas and promptly reports to supervisors; promotes safety by working as safely and efficiently as possible; consistently follows infection control and universal precautions and other guidelines; identifies and corrects or reports hazardous conditions to supervisors; understands facility emergency and evacuation protocols.

Leadership Skills:

Possesses an approachable management style; ability to solve problems and treat each situation fairly; is calming during a crisis situation.

Customer Service:

Treats all residents, families, co-workers, inquiries, referral sources and supervisors with dignity and respect and takes initiative to exceed customer expectations; professionally represents the facility, self and position to visitors and residents; shows genuine concern for the residents; proactively identifies needs of the physical plant; demonstrates a positive attitude and willingness to help to all visitors and residents; remains sensitive to all needs through listening, observing and responding appropriately for the position held.

Essential Job Duties:

1. Assists the Social Worker in overall supervision of the Social Services Department to ensure the social, emotional and medical needs of the residents are met in accordance with all laws, regulations and facility guidelines.

2. Accurately and timely documents all conversations and other interactions with the residents and families as it relates to the care plan and admission assessments and in accordance with all state laws and regulations.

3. Identifies risk factors for psychosocial deterioration and intervention responses by analyzing documentation and other data.

4. Enhances the quality of resident lives by providing interventions which assist residents in reaching goals and addressing their social, emotional and physical needs.

5. Plays a key role in implementation of advance directives decision processes.

6. Develops a consistent schedule for family council meetings.

7. Coordinates a formal grievance or complaint process for residents and families.

8. Actively participates in the new resident orientation program.

9. Assists the residents and families with change management aspects of becoming a new resident through individual and group facility programs.

10. Performs discovery meetings with residents and families to learn more about the resident’s social history, interests and hobbies.

11. Provides counseling with residents and families to determine feelings, fears and anxieties about death or dying, emotional, physical, spiritual and intellectual needs.

12. Promotes family involvement and develops a supportive family network.

13. Acts as liaison for social, health and community agencies by providing individual or group assistance to the residents through crisis and change management.

14. Recruits, selects and orients qualified team members and volunteers for the Social Services Department.

15. Provides expectations and monitors performance of Social Services Department staff.

16. Provides overall supervision of the Social Services Department and provides guidance to the Social Services staff.

17. Provides information to families regarding community resources for financial needs and to promote emotional stability.

18. Builds individual rapport with families to promote communication.

19. Participates in various routine and non-routine meetings to enhance the effectiveness and efficiency of facility outcomes.

20. Assists with the process of discharge planning.

21. Supports the development of goals and objectives by assisting with social assessments and care plans that identify the medical-related social and emotional needs of the residents.

22. Promotes family involvement in setting goals and objectives for the residents.

23. Educates residents and families on the importance of following medical recommendations related to care.

24. Assists in scheduling appointments and transportation for the residents.

25. Completes forms timely and accurately in accordance with state, local and facility guidelines.

26. Assists in coordination of care plan meetings with families.

27. Assists in resolution of family or resident grievances.

28. Coordinates room changes as needed.
