Job Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and home care environments. Your expertise will support patient recovery, comfort, and well-being through comprehensive assessments, medication administration, and personalized care plans. This position offers an exciting opportunity to make a meaningful difference in the lives of diverse patient populations while working in a fast-paced, team-oriented environment.

Responsibilities

Conduct thorough patient assessments, including vital signs, physical examinations, and medical history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
Administer medications and injections accurately following physician orders and safety protocols, including IV infusions and catheter care.
Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
Manage airway maintenance and ventilator support for patients requiring respiratory assistance.
Provide specialized care for patients with complex conditions such as Alzheimer’s disease, dementia, or memory impairments in settings like memory care or hospice.
Support acute pain management strategies and coordinate case management efforts for long-term or post-acute care patients.
Collaborate with multidisciplinary teams to develop and implement individualized care plans that adhere to long-term care regulations and HIPAA confidentiality policies.
Educate patients and their families on health coaching, medication administration, discharge planning, and disease management techniques.
Maintain accurate medical documentation of patient care activities while ensuring compliance with ICD coding standards (ICD-9/ICD-10).
Respond effectively to triage situations in urgent or emergency care settings such as emergency rooms or Level I/II trauma centers.

Experience

Proven experience working as an LPN in diverse healthcare environments including hospitals (especially ICU or trauma centers), nursing homes, skilled nursing facilities, or outpatient clinics.
Critical care experience with familiarity in acute care procedures like spinal taps, suturing, or sterile processing is highly desirable.
Experience working with individuals with disabilities or developmental challenges, including those requiring behavioral health support or specialized dementia/cognitive impairment care.
Proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks for documentation and patient record management.
Knowledge of infection control protocols and aseptic techniques essential for maintaining clinical safety standards.
Ability to perform advanced skills such as phlebotomy, venipuncture, tube feeding management, catheterization, and first aid procedures confidently.
Prior experience in home & community care settings or travel nursing is a plus for adaptable candidates eager to serve varied populations across different environments.
