THE OPPORTUNITY
Vizx Corp is conducting a confidential search for an Executive Director to lead an established, NYSDOH-regulated adult care services organization in the New York City metropolitan area. This is a true executive appointment — full P&L accountability, NYS regulatory authority, and strategic leadership of a multi-disciplinary organization serving one of the most diverse urban populations in the country.
The Executive Director will serve as the senior-most on-site leader, reporting directly to the Vizx Corp CEO. You will oversee programs that may include a New York State Adult Care Facility (ACF), Assisted Living Program (ALP), Assisted Living Residence (ALR), or MLTC-contracted home-based services — and you will be expected to navigate NYSDOH regulatory requirements with executive command and compliance rigor.
This role is for an executive who has operated within New York State's healthcare regulatory environment — not someone learning it. The NYC market demands fluency in MLTC contracting, 1199SEIU or DC37 union labor relations, and the complex multilingual community dynamics of the five boroughs and surrounding metro area. SCOPE OF LEADERSHIP
This is a full organizational leadership role. The Executive Director oversees:
1. Clinical & Program Operations
- ACF, ALP, ALR, and/or MLTC-contracted program delivery
- MDT structure: physicians, RN/LPN, social workers, care managers
- Direct care worker and caregiver recruitment, training, and evaluation
- NYSDOH clinical assessments, prior authorization, and MLTC care plan documentation
2. Financial & Administrative Oversight
- Full P&L accountability for the organization
- NYS Medicaid and MLTC revenue cycle: billing, claims, AR aging
- Budget management, vendor contracts, and cost controls
- NYSDOH rate-setting compliance; financial reporting to Vizx Corp
3. Regulatory & Compliance Authority
- NYSDOH compliance: 18 NYCRR (ACF), 10 NYCRR (health facilities)
- ACF Operator License or ALP Certification maintenance and survey readiness
- NYS Justice Center and SCR compliance for all staff and volunteers
- MLTC contract compliance; OPWDD requirements as applicable
4. Workforce & Organizational Leadership
- Executive leadership across all NYSDOH-regulated programs and staff
- Union labor relations (1199SEIU and/or DC37) — CBA compliance, grievance management
- Talent strategy for a high-demand, multilingual NYC workforce
- Organizational transition leadership in partnership with Vizx Corp
KEY RESPONSIBILITIES
1. Clinical Operations & Service Delivery
- Lead delivery of services across all program lines with full accountability for member outcomes, caregiver quality, and program integrity
- Maintain the multidisciplinary team (MDT) structure required under applicable regulations — ensuring each function is appropriately staffed, supervised, and performing
- Oversee caregiver and direct care worker recruitment, onboarding, annual evaluation, and ongoing support to sustain program quality and member safety
- Ensure timely completion of all clinical documentation: assessments, prior authorizations, care plans, and discharge planning
- Monitor clinical quality indicators and drive continuous improvement across all care delivery programs
2. Regulatory Compliance & Risk Management
- Maintain full compliance with NYSDOH regulations governing Adult Care Facilities (18 NYCRR Parts 485–487), Assisted Living Programs (18 NYCRR Part 494), and all applicable NYS and NYC program requirements
- Own and maintain the NYSDOH Adult Care Facility Operator License or ALP Certification, including annual renewal, survey readiness, and plan of correction submissions
- Oversee NYS Justice Center compliance — fingerprinting, SCR clearance, and reportable incident obligations for all staff, contractors, and volunteers
- Ensure compliance with MLTC plan contracts — service delivery standards, utilization management, and quality reporting requirements
- Maintain mandated reporter training and compliance under New York Social Services Law 413 for all applicable staff
- Monitor NYSDOH, NYSDSS, NYC DFTA, and OPWDD regulatory developments and implement policy updates proactively
- Manage union labor relations compliance (1199SEIU and/or DC37 CBA obligations) — grievance procedures, wage schedules, and benefit administration
- Ensure HIPAA compliance and all data governance obligations under MLTC and NYSDOH contractor agreements
3. Financial Leadership
- Own the organization's P&L — managing budget, cost structure, and financial performance to margin and growth targets set with Vizx Corp leadership
- Oversee revenue cycle performance: billing accuracy, claims submissions, AR aging, prior authorization compliance, and payer reconciliation
- Identify and execute opportunities for financial efficiency, service-line growth, and sustainable scaling
- Deliver regular, board-quality financial performance reporting to Vizx Corp leadership
4. Workforce Leadership & Organizational Culture
- Lead, develop, and retain a high-performing executive and clinical team — with clear expectations, ongoing feedback, and visible accountability
- Own talent strategy: workforce planning, succession readiness, and key-person risk mitigation in a competitive labor market
- Build and maintain a culture of mutual respect, clinical excellence, and organizational pride across a diverse workforce
- Serve as a stabilizing, accessible executive presence for staff, caregivers, and community partners throughout organizational transition
5. Stakeholder & Community Relations
- Build and maintain referral relationships with NYC hospitals, SNFs, discharge planning teams, PCPs, social workers, and community-based organizations across the five boroughs and surrounding metro area
- Serve as the primary executive representative with NYSDOH, NYC DFTA, and contracted MLTC plans (VNS Choice, Centers Plan, MetroPlus, RiverSpring, ElderServe)
- Build relationships with PACE programs, NYC HARP plans, and OPWDD service coordinators relevant to the organization's program mix
- Engage with culturally specific community organizations across Spanish, Chinese, Russian, Bengali, and other primary language communities throughout NYC Metro
- Maintain productive, professional relationships with 1199SEIU and DC37 union representatives, including regular labor-management meetings
- Communicate proactively with members, families, and caregivers in their primary language or through qualified interpreter services
6. Organizational Transition & Integration
- Partner with Vizx Corp's Director of Administration & Operational Systems on system onboarding, policy harmonization, and operational alignment
- Build the executive team and operational infrastructure required for sustained performance at scale in the NYC Metro, NY market
- Develop and execute a 12-month strategic roadmap anchored in quality outcomes, regulatory compliance, and program growth
QUALIFICATIONS & LICENSURE
Licensure & Background Check Requirements — NYC Metro, NY
- An active NYSDOH Adult Care Facility Operator License is required if this role involves direct oversight of a licensed ACF or ALP. Candidates must be eligible to hold or transfer this license under NYS DOH requirements. Candidates holding an LNHA (Licensed Nursing Home Administrator), LCSW, LMHC, RN, or CCM credential are strongly preferred. All candidates must complete NYS Justice Center fingerprinting, NYSDOH criminal history review, and SCR clearance prior to hire.

- Mandated Reporter: New York State Mandated Reporter (Social Services Law 413) — training and certification required
Required Qualifications
- 7+ years of progressive healthcare leadership, with at least 3 years in an executive or senior director role in New York State
- Demonstrated P&L ownership in a NYSDOH-regulated, NYS Medicaid-contracted, or MLTC-contracted adult care or home-based services organization
- Direct experience operating within NY State Adult Care Facility (ACF, ALP, or ALR) or MLTC regulatory frameworks
- Track record of leading organizations through NYSDOH surveys, MLTC audits, or equivalent regulatory reviews
- Financial fluency in NYS Medicaid billing, MLTC rate-setting, and healthcare revenue cycle management
- Demonstrated experience managing union labor relations (1199SEIU and/or DC37) in a regulated healthcare environment
- Exceptional communication and relationship management skills across NYSDOH regulators, MLTC plans, union representatives, staff, and executive leadership
Preferred / Differentiating
- NYSDOH Adult Care Facility Operator License (active or recent) — required if role involves oversight of a licensed ACF or ALP
- Prior Executive Director, CEO, or Administrator role at a NYSDOH-licensed ACF, ALP, ALR, certified home health agency (CHHA), or MLTC-contracted organization
- Familiarity with MLTC plan contracting, OPWDD HCBS waiver services, NHTD/TBI waivers, or NYC HARP program requirements
- Experience supporting corporate acquisition, NYSDOH change-of-ownership (CHOW) processes, or organizational restructuring
- Master's degree in Healthcare Administration, Business Administration, Public Health, or related field (MHA, MBA, or MPA preferred)
- ACHE Fellow (FACHE), LNHA (Licensed Nursing Home Administrator), LCSW, LMHC, or CCM credential
- Bilingual: Spanish, Cantonese, Mandarin, Russian, or Bengali strongly preferred given the NYC Metro service population
- NYS Justice Center clearance and SCR clean record (required prior to hire; no disqualifying offenses under NY Social Services Law)
