Now Hiring: Care Coordinator — Edmond/OKC

Grace Care is expanding into the Edmond/OKC area and looking for a dependable Care Coordinator to help launch and run this new location!

Training Note: Before this location opens, candidates must be willing to travel to Enid two days a week for training. Travel time and mileage will be paid. Starting hours will be limited during this initial training period.

Schedule: Monday–Friday, 8:30 AM–3:30 PM (must be available for occasional after-hours calls and home visits)

Position Summary

Grace Care Home Care for Seniors is seeking a dedicated, experienced Care Coordinator to join our leadership team. This key role ensures the delivery of high-quality care by supervising caregivers in the field, mentoring staff, supporting professional development, assisting with hiring and onboarding, and signing on new clients.

The ideal candidate is a natural leader who guides caregivers with compassion and accountability, communicates effectively with clients and staff, and provides consistent support to drive excellent care outcomes.

Key Responsibilities

Supervision & Mentorship

Conduct routine supervised home visits to observe and evaluate caregiver performance
Provide real-time coaching and support to uphold care quality standards
Document observations and share feedback with management
Maintain strong knowledge of Grace Care's services to inform potential clients and families

Client Care Coordination

Work closely with our nurse to organize and coordinate the start of care for new clients
Communicate any changes in client care needs, plans, or status to the appropriate team members
Ensure a smooth, well-coordinated transition as clients begin services

Training & Onboarding

Participate in interviewing and onboarding new caregivers
Lead caregiver orientation and hands-on training
Meet weekly with new hires during their first 90 days to support their success
Facilitate smooth introductions between caregivers and clients
Help lead quarterly staff meetings

Performance Management & Compliance

Address performance concerns with professionalism and clarity
Administer timely corrective actions or disciplinary measures as needed
Ensure caregivers adhere to agency policies, procedures, and client care plans
Conduct 90-day and annual performance reviews

Collaboration & Communication

Serve as a liaison between caregivers, clients, and the internal care coordination team
Work closely with the Scheduling Coordinator to ensure optimal caregiver-client matches
Respond promptly to caregiver and client concerns
Maintain clear, timely documentation of visits, issues, and outcomes
Engage in direct, constructive conversations both in person and by phone
