We are seeking an experienced, compassionate, and self-motivated Licensed Practical Nurse (LPN) residing in New Hampshire to join our growing remote care management team. This is an excellent opportunity for nurses who enjoy building relationships with patients, coordinating care, and making a meaningful impact from the comfort of their home office.
Position Summary
As a Remote Care Management Nurse, you will provide telephonic care coordination and support for Medicare patients with multiple chronic conditions at our New Hampshire clinic location. You will work closely with patients, providers, caregivers, and interdisciplinary healthcare teams to improve patient outcomes, increase engagement, and ensure compliance with CMS Chronic Care Management requirements.
This is a fully remote 1099 contractor position available to New Hampshire residents.
Duties
- Conduct monthly telephonic patient outreach and care management activities
- Complete comprehensive patient assessments and ongoing health reviews
- Develop, implement, and update individualized care plans
- Perform medication reconciliation and medication adherence reviews
- Identify barriers to care and connect patients with appropriate resources
- Coordinate care with primary care providers, specialists, caregivers, and community resources
- Educate patients regarding chronic disease self-management
- Monitor patient progress toward care plan goals
- Accurately document all patient interactions and interventions in electronic health record systems
- Maintain compliance with CMS guidelines and documentation requirements
- Participate in team meetings, training sessions, and quality improvement initiatives
- Escalate urgent clinical concerns according to established protocols
Required Qualifications
- Resident of New Hampshire
- Active and unrestricted LPN license
- Minimum of 2 years of nursing experience
- Previous Chronic Care Management (CCM), Case Management, Care Coordination, Population Health, or Long-Term Care experience
- Strong computer and technology skills
- Excellent communication and patient engagement abilities
- Ability to work independently in a remote environment
- Reliable internet connection and dedicated home office workspace
Preferred
- Previous remote nursing experience
- Experience with Medicare care management programs
- Experience using EHR/EMR platforms
- Long-term care, primary care, geriatrics, or population health experience
