Roles and Responsibilities

Navigation:

Maintain certification and knowledge, expertise and follow the eligibility, enrollment and program Specification for PVHC, Health Insurance Marketplace (Affordable Care Act –open and special enrollment periods) and Your Texas Benefits to facilitate the selection of plans by clients.
Participate in the Community Partner Program ongoing weekly trainings and trainings provided by TACHC to maintain knowledge and certification in YTB and ACA.
Follow the Navigation Workflow to access clients to clinical intervention and treatment.
Register, collect and input client data into specific clinical databases.
Maintain client registration, ACA, YTB, and CPW caseload and provide and track follow-up.
Collaborate with all partners, including external site partners, internal programs, and clinic team, to provide navigation services to targeted audiences and connect their clients to PVHC and other social services via a referral linkage.
Maintain fidelity of the PVHC discount program registration.
Adhere to the Access Navigation PVHC discount program manual, and Program Handbook.

Auditing policies and procedures.

Participate in personal and professional development training provided by leadership for personal and professional development for the overall success of the program.
Ensure the protection and security of personal identifiable, confidential information in a professional and responsible manner (HIPAA laws and guidelines) and other dutiesb assigned by supervisor
Collect data and generate reports for evaluation, program performance measures, and continuous quality improvement through data collection tools and strategies

Case Management for Children & Pregnant Women

Identify potential clients through different activities, including implementation and community engagement.
Conduct client assessments to identify health and social needs, barriers, and appropriate support services.
Develop, implement, and monitor individualized service plans to help clients achieve their goals.
Coordinate referrals and connect clients with community resources, healthcare providers, educational programs, and social services.
Maintain accurate and timely case notes, client records, and required documentation to ensure compliance with program and funding requirements.
Provide ongoing follow-up, advocacy, and support to clients while monitoring progress and adjusting service plans as needed.
Follow appropriate billing and documentation procedures for each encounter to ensure reimbursement for services.

Outreach and Education:

Participate in and support community outreach activities, including health fairs, presentations, and educational events.
Organize and collaborate with community partners to promote awareness of Project Vida services and initiatives.
Conduct outreach and education through presentations, health education, and seasonal campaigns, including door-to-door canvassing and distribution of informational materials.
Conduct outreach and education in a manner that matches the linguistic and cultural needs of the participants and community. Build health literacy and cross-cultural communication.
Represent the organization at community events to connect individuals with available health and social services.
Conduct Family Planning Survey as a means to assess clients needs.
